摘 要:目的 观察钙通道阻滞剂对未成熟心肌的保护效果。方法 使用改良的非再循环式Langendorff离体心灌注装置 ,比较未使用盐酸尼卡地平和分别在缺血前、低温缺血停跳时、复灌初期 3个不同时间给与盐酸尼卡地平者的心功能和心肌酶变化 ,测定缺血后心肌ATP、总钙含量、心肌含水量 ,观察心肌和冠状动脉内皮细胞结构的变化。结果 实验组的各项指标及对心肌及血管内皮结构保护明显优于对照组 ,实验组间差别无显著性意义。结论 使用钙通道阻滞剂可以对未成熟心肌产生良好的保护作用。Objective: To observe whether nicardipine hydrochloride has effect on immature myocardial protection against reperfusion injury in isolated rabbit hearts. Methods: On modified Langendorff apparatus, 32 isolated hearts of immature rabbit (3~4 weeds of age, 350~450?g of weight) were randomly divided into 4 groups (n=8). Hypothermic ischemic contrast group was Group I; nicardipine was administrated before, during myocardial ischemia and while reperfusion respectively in Group II to Group IV. The dose of nicardipine was 3 μg. 37℃ Krebs Henseleit solution and 4℃ St.Thomas solution were used. The ischemic duration was 90 minutes, the local temperature was 15℃, and the hearts were observed for 60 minutes after recovery of infusion. Hemodynamics variables, myocardial water content and the leakage of myocardial enzyme, myocardial ATP and calcium content were examines. Myocardial and endothelial structure was observed. Results: (1) Myocardial function: the recovery of LVSP and ±dp/dt in group II were significantly higher than that in group I, but the recovery of LVEDP was significantly lower than that in group I (P<0.01). The recovery of LVSP and LVEDP in group II was better than that in group III and group IV, but there was no significant difference between two groups (P>0.05). (2) Myocardial enzyme: the leakage enzyme in experimental groups was significantly lower than that in contrast group (P<0.05 or P<0.01). (3) Myocardial calcium in experimental groups were lower obviously than that group I(P< 0 05 ). (4) Myocardial ATP in experimental groups were higher than that in group I(P<0.05 or P>0.01), but there was no significant difference among these groups (P>0.05). (5) Myocardial water content: there was no significant difference among groups (P<0.05). (6) Myocardial structure: the optimal group was group II which had the best protective effect on myocardial structure and endothelium of coronary artery.
