超声心动图对先天性冠状动脉瘘的诊断价值

摘　　要：目的 :总结并探讨先天性冠状动脉瘘的超声心动图特征及超声心动图对该病的诊断价值。方法 :利用二维超声 (2 DE)、脉冲多普勒 (PW)、彩色多普勒血流显象 (CDFI)检测受累冠状动脉开口、内径、追踪其走行至瘘口 ,以及瘘口处血流特点。结果 :本组 36例患者均经手术证实 ,超声检出 2 9例 ,经冠状动脉造影和 /或手术发现 7例 ,超声诊断与手术和 /或冠状动脉造影相符 2 7例 ,部分相符 2例。敏感性为 80 .6 % ,特异性 10 0 %。共检出右冠状动脉瘘 18例 ,左冠状动脉瘘 14例 ,左右冠状动脉瘘 4例。结论 :超声心动图对先天性冠状动脉瘘具有较高的诊断价值 ,可作为首选诊断方法 ,但对检出冠状动脉内径正常、瘘口较小的患者有一定的困难。Objective:To summarize the characteristics and to investigate the diagnostic value of echocardiography in patients with congenital coronary artery fistula.Methods:Coronary artery fistula opening and interior diameter were measured.The fistulae were traced and the blood flow characteristics were observed using 2 D echocardiography, pulsed-wave Doppler and color Doppler flow imaging.Results:All 36 patients with congenital coronary artery fistula were validated by surgery.Twenty nine cases were diagnosed echocardiographically and the others by surgery and/or coronary artery angiography Twenty seven patients' echocardiogram results were consistent with surgery and/or coronary angiograms,and 2 patients were partly consistent The sensitivity of echocardiography in diagnosis was 80 6%, and the specificity was 100% in this group Eighteen patients manifested right coronary artery fistula, 14 left coronary artery fistula, and 4 both.Conclusions:Echocardiography shows high value in diagnosing congenital coronary artery fistula,and can be as the primary method for diagnosis.However,it is still difficult to examine the patient with normal interior diameter of the coronary arterie and with small fistula opening.
