摘 要:目的 :研究肝癌、癌旁肝硬化及病毒性肝炎 3种病变中c myc、bcl 2基因蛋白和p63的表达特征 ,探讨其对肝癌发生的影响。方法 :采用免疫组化 (S P)法检测石蜡包埋组织中c myc、bcl 2和p63的表达。 结果 :c myc、bcl 2蛋白和p63均可表达于胞质和胞核中 ,c myc在 3种病变中的阳性表达率与表达强度均无差异 ;bcl 2在 3种病变中的阳性表达率虽无差异 ,但在癌旁肝硬化中 ,其强阳性率高于肝癌与慢性病毒性肝炎 ,并在肝癌低分化组阳性表达呈降低趋势 ;p63阳性主要表达于癌旁肝硬化与肝癌中 ,并高于慢性病毒性肝炎 ;c myc、bcl 2和 p63在肝癌中阳性表达的相关性检验显示c myc与bcl 2阳性率呈正相关(r =0 6742 )。结论 :c myc在慢性病毒性肝炎、癌旁肝硬化及肝癌中的持续表达可引起肝细胞增殖和选择性的转化导致癌变。bcl 2的抗凋亡作用在肝癌形成早期并与c myc具有协同作用。Purpose To investigate the effect of bcl 2,c myc,and p63 proteins in the primary hepatocellular carcinoma (PHCC) through their expressions in three groups, PHCC,pericarcinoma cirrhosis and chronic viral hepatitis(CVH). Mathods The expressions of bcl 2,c myc and p63 proteins were evaluated in paraffin embedded tissue by immunohistochemical techniques (S P). Results The positive expressions of bcl 2,c myc and p63 were located in cytoplasm and nuclear. There were no significant different in positive rate and various strong expressions of c myc among these three groups. Although no significant difference of bcl 2 positive rate was detected in these three groups, stronger positive rate of expressions was prominently higher in pericarcinoma cirrhosis than in PHCC and chronic viral hepatitis, respectively. And, bcl 2 expression reduced orientation with poorer differentiated of PHCC. p63 was chiefly expressed in both pericarcinoma cirrhosis and PHCC, and the positive rate in both groups was significantly higher than that in chronic viral hepatitis. There was a significant positive relationship between c myc and bcl 2 expressions in PHCC ( r = 0.674 2 ). Conclusions In these three groups, continued expression of c myc may induce the proliferation of hepatocytes and selected transformation, and promote carcinogenesis. Anti apoptosis of bcl 2 may occur in early stages of PHCC, in cooperation with c myc. p63 may promote transformation of PHCC.
