摘  要:目的 观察参附注射液对体外循环 (CPB)中肿瘤坏死因子 - α(TNF- α)和白细胞介素 - 6 (IL- 6 )的影响。 方法 将 2 0例人工心脏瓣膜置换术患者随机分成参附组和对照组 (每组 10例 )。参附组分别于麻醉诱导前、CPB前及主动脉开放 10分钟内分别静脉滴注参附注射液 2 0 ml、4 0 ml和 4 0 ml,于 CPB前、主动脉阻断前、主动脉阻断 30分钟、主动脉开放 15分钟和 6 0分钟时采用放射免疫法测定两组血浆中 TNF- α和 IL- 6的含量。 结果 主动脉阻断前、主动脉阻断 30分钟、主动脉开放后 15分钟和 6 0分钟 ,对照组 TNF-α值均明显高于参附组 (P<0 .0 5 ) ,各时点 IL -6组间比较差别无显著性意义 (P>0 .0 5 )。 结论 参附注射液可降低主动脉开放后血浆 TNF- α的含量 ,具有对抗CPB所致的炎性反应的作用 ,而对 CPB中 IL- 6作用不明显。ObjectiveTo investigate the effects of Shengfu injection on tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6) during cardiopulmonary bypass(CPB). MethodsTwenty patients were randomly divided into Shengfu group and control group(10 in each group). In Shengfu group, intravenous drip of 20 ml , 40ml, 40ml of Shengfu injection were given before anesthesia, before CPB, and 10 min after aortic off-clamping, respectively, while in control group, 0.9% NaCl solution was given instead of Shengfu injection as a placebo . Blood samples were taken before CPB, before aortic cross-clamping, 30 min after aortic cross-clamping, 15 min and 60 min after aortic off-clamping. Blood samples were taken for measuring TNF-α and IL-6 by radio-immunoassay in two groups. ResultsBefore aortic cross-clamping, 30 min after aortic cross-clamping, and 15min and 60min after aortic off-clamping, TNF-α in control group was significantly higher than that in Shengfu group (P<0.05). IL-6 in both group showed no obvious difference in various time points (P>0.05). ConclusionsShengfu injection could lessen the release of TNF-α in blood after aorta is off-clamped, and reduce the inflammatory response resulted from CPB. Shengfu injection has no obvious effect on IL-6 during CPB.
