机械通气治疗重度有机磷农药中毒中间综合征的探讨

摘　　要：目的　探讨急性有机磷农药中毒 (AOPP)中间综合征 (IMS)患者机械通气治疗的时机及对预后的影响 .方法　对 5 4例 IMS患者随机分成即刻通气组与延时通气组 .通过观察 Pa O2 ,Sa O2 ,Pa CO2 ,呼吸频率、存活率、死亡率和恢复时间等指标 ,比较即刻与延时使用机械通气的疗效 .结果　即刻通气比延时通气 Pa O2 ,Sa O2 和 Pa CO2 明显改善 ,死亡率明显降低 ,恢复时间明显缩短 ,存活率提高 (P<0 .0 5 ) ,呼吸频率无明显差异 (P>0 .0 5 ) .结论　重度有机磷中毒患者在出现 IMS症状时 ,应尽早使用机械通气治疗 ,即刻通气较延时通气预后好 .AIM To study effects of using mechanical venti lation in organphosphorus pesticides poisoning patients with intermediate syndrome. METHODS Fifty four patients were divided into immediate group and delayed group using MV randomly, PaO 2, SaO 2, PaCO 2, breath rate, live rate, death rate, recovering time were observed, and curative effect was compared between two groups. RESULTS Compared with MV delayed group, immediated group, PaO 2, SaO 2, PaCO 2 were improved, death rate decrcased and recovering time shortened significantly, live rate increased ( P < 0.05 ). Breath rate had no significant difference ( P >0.05). CONCLUSION Organphosphorus pesticides poisoning with IMS should be ventilated as early as possible, MV immediate used groups has better curative effect than delay used groups.
