摘 要:目的 :通过MRI图像研究无脊柱脊髓畸形的成人人群圆锥位置的变化。方法 :用 1.5TMRI机使用T1加权、正中矢状位自旋回波扫描 ,对 1179例病人圆锥末端的位置进行观察。根据其与相应椎体的上、中、下 1 3及椎间盘的对应关系定位。结果 :圆锥末端的平均位置为L1的下 1 3(范围 :T12的中 1 3~L3的上 1 3) ,圆锥末端的位置呈正态分布 ,女性圆锥位置低于男性 (P <0 .0 0 1) ,各年龄段无差别 (P >0 .0 5 )。结论 :本组成人人群中圆锥末端的位置从T12的中 1 3~L3的上 1 3,女性圆锥位置低于男性 ,对麻醉和手术有重要意义。Objective:To determine the variation in position of the conus medullaris in a living adult population without spinal deformity through MR imaging.Methods: The T1-weighted, midline,sagittal,spinecho magnetic resonance imaging studies of 1179 patients were assessed to identify the tip of the conus medullares by 1.5 T MRI machine. This location was recorded in relation to the upper, middle,or lower third of the adjacent vertebral body or the adjacent intervertebral disc. Results:The mean conus position was the lower third of L1(range: middle third of T12 to upper third of L3).The variation in conus positions followed a normal distribution. The conus position in female patients was significantly lower than that of male counterparts. No significant difference in conus position was seen with increasing age. Conclusion: Our study of the largest adult population showed the distribution of conus location to range from the middle third of T12 to the upper third of L3, female population have a much lower conus position than that of male ones, which gives significant importance to anesthesia and operation.
