摘 要:目的 本文就以氦氧混合气 (79%氦 - 2 1%氧 )为驱动气源雾化吸入 β2 受体激动剂对支气管哮喘的治疗作用进行探讨。方法 选择门诊 2 4例轻中度支气管哮喘患者 ,随机分为治疗组 (13例 ,以氦氧混合气为驱动气源 )和对照组 (11例 ,以压缩空气为驱动气源 )雾化吸入备劳特 ,观察治疗前后肺功能的变化 ;选择病房 8例重症支气管哮喘患者采用自身对照方式 ,观察以氦氧混合气为驱动气源和以压缩空气为驱动气源雾化吸入备劳特前后肺功能和血气分析的变化。结果 门诊哮喘患者治疗组与对照组比较除PEF有显著性差异外 ,主要肺功能指标无统计学意义 ;病房重症哮喘患者以氦氧混合气为驱动气源与以压缩空气为驱动气源疗效比较 ,FVC、FEV1 0 、FEF50 差异具有显著性意义 ,血气分析结果示PaO2 差异具有显著性差异 ,PaCO2 有下降趋势。结论 氦氧混合气与雾化吸入β2 受体激动剂在治疗重症支气管哮喘方面与传统方法比较有明显优势 。Objective To evaluate the effectieness of a helium oxygen mixture (79%He 21%O 2)as an aerosol compressed air for β 2 agonist therapy in patients with exacerbation of asthma.Methods A prospective,single blinded study was performed in outpatient department and hospital ward.24 patients in outpatient department with mild to moderate exacerbation of asthma were enrolled.The patients were randomly assigned to experimental group(13 cases)and controlled group(11 cases).The experimental group inhaled Berotec with heliox driven,and the controlled group inhaled Berotec with compressed air driven.8 patients in hospital ward with severe exacerbation of asthma were enrolled.The patients inhaled Berotec with heliox or compressed air driven in a random order.The results of lung function test and arterial blood gas analysis were recorded.Results In the outpatients,there were no obvious differences between two groups for FVC,FEV 1 0 and FEV 50 .But in the severe asthmatic patients in hospital ward,there were significant differences between heliox driven and compressed air driven for FVC,FEV 1 0 ,FEF 50 and PaO 2.Conclusion Compared with the traditional way for compressed air driven,the way of inhaling β 2 agonist with heliox driven has obvious advantage for severe asthma.The results suggest a synergistic effect between heliox and β 2 agonist in the treatment of exacerbation of severe asthma.
