血小板减少症患儿血浆血小板生成素的研究

摘　　要：探讨血小板减少症患儿血浆血小板生成素 (throm bopoietin,TPO)水平 ,及其与外周血血小板计数 (plateletcounts,PL T)的关系。采用 EL ISA法检测初治急性特发性血小板减少性紫癜 (acute idiopathic thrombocytopenic purpu-ra,AITP) 13例、再生障碍性贫血 (aplastic anemia,AA) 4例、急性淋巴细胞白血病 (acute lymphoblastic leukemia,AL L) 9例的血浆 TPO、血小板相关抗体 Ig G(platelet- associated Ig G,PAIg G)水平。结果 :AITP、AA、AL L 患儿血浆 TPO水平均较正常对照组升高 ,尤以 AA和 AL L 升高明显 ,且后两者与外周血血小板计数呈显著负相关。表明 :不同病因的血小板减少症血浆 TPO水平不同 ,外周血血小板数是血小板减少症患儿血浆 TPO水平重要的反馈调节因素之一。 TPO水平的检测有助于临床鉴别血小板减少症的病因及指导治疗。To investigate the relationships between the levels of plasma thrombopoietin (TPO) and platelet counts (PLT) in children with thrombocytopenia, the plasma TPO levels were determined by a sensitive sandwich ELISA, the PAIgG by a ABC ELISA. Results showed the plasma TPO levels in acute idiopathic thrombocytopenic purpura (AITP), aplastic anemia (AA) and acute lymphoblastic leukemia (ALL) children were all elevated compared to those in normal controls, especially in AA and ALL children. The serum TPO levels in the children with AA, ALL was inversely correlated with PLT. It was suggested that the thrombocytopenia with different causes had different plasma TPO levels. The peripheral blood PLT was one of important feedback regulating factors for serum TPO levels in the children with thrombocytopenia. Plasma TPO level measurements have certain diagnostic value in thrombocytopenia.
