血浆置换治疗小儿重症特发性血小板减少性紫癜的临床研究

摘　　要：目的　通过对血浆置换 (PE)与静脉注射大剂量丙种球蛋白 (HD IVIG)治疗小儿重症特发性血小板减少性紫癜 (ITP)的疗效对比观察 ,选择最佳治疗方法。方法　选择小儿重症ITP 38例 ,在常规使用抗生素、维生素C、能量合剂的基础上 ,治疗组予PE治疗 ,对照组予HD IVIG治疗 ,观察临床疗效及血小板恢复正常时间。结果　治疗组与对照组在治疗效果、血小板恢复正常时间上均无显著差异(P均 >0 .0 5 )。结论　PE疗法效果肯定 ,安全经济 ,是治疗小儿重症ITP的最佳选择。Objective To investigate the therapeutic effects of plasma exchange technique(PE) and high dose intravenous gammaglobulin (HD IVIG) on child severe idiopathic thrombocytopenic purpura (ITP).Method 38 cases of children with severe ITP were divided randomly into the treated group and the control group.All of the patients were treated with antibiotics,Vitamin C and enrgy mixture.In addition,the treated group were treated with PE and the control group with HD IVIG.Clinical effectiveness and time of platelet count rising to normal level in the two groups were observed.Result There was no significant difference between the two groups in clinical effectiveness and time of platelet count to normal level(P>0.05).Conclusion PE technique is effective and safe in the treatment of children with severe ITP.
