摘　　要：目的探讨医院-社区-家庭联动管理在支气管哮喘患儿中的应用与效果。方法选择2016—2017年在上海市浦东新区联洋社区接受治疗的支气管哮喘患儿90例作为研究对象,以2016年1—6月收治的45例患儿作为对照组,以2017年1—6月收治的45例患儿为观察组。对照组接受常规社区治疗护理、健康教育及随访,观察组采用医院-社区-家庭联动管理模式。比较两组患儿入组时、干预3个月和6个月后的肺功能指标,以及6个月随访期间哮喘急性发作次数、再就医和再入院的次数。结果观察组患儿肺功能指标(PEF%及FEV1%)改善优于对照组(P<0.05),且随访期间的哮喘急性发作次数、再就医和再入院次数均少于对照组(P<0.05)。结论医院-社区-家庭联动管理能合理利用医疗资源、改善哮喘患儿的肺功能、降低哮喘急性发作频次,对促进儿童哮喘的控制有积极作用。Objective To explore the application effect of hospital-community-family collaborative management in children with bronchial asthma.Methods A total of 90 children with bronchial asthma who received treatment in Lianyang Community,Pudong New Area,Shanghai from 2016 to 2017 were selected as study subjects.45 children who were admitted from January to June in 2016 were enrolled into the control group,and 45 children who were admitted from January to June in 2017 were enrolled into the observation group.Routine treatment and care,health education and follow-up by the community were given in control group,whereas hospital-community-family collaborative management mode was utilized in observation group.The pulmonary function measures at enrollment and after 3 and 6 months of intervention,and the number of acute asthma attacks,medical re-visits,and re-admissions during the 6-month follow-up period were compared between the two groups.Results The improvement of pulmonary function measures(PEF%and FEV1%)in the observation group was better than that in the control group(P<0.05),and the number of acute asthma attack,medical re-visits,and re-admissions during the follow-up period were all less than those in the control group(P<0.05).Conclusion The hospital-community-family collaborative management can make rational use of medical resources,improve the pulmonary function of children with asthma,reduce the frequency of acute asthma attacks,and play a positive role in promoting the asthma control for children.
