摘 要:目的通过对不同病程高调耳鸣患者DPOAE反应幅值的结果分析,了解病程长短与DPOAE反应幅值之间的关系。方法按病程长短将听力正常耳鸣患者分为三组,第一组(病程≤3个月),27人,31耳,年龄16-62岁(38.57±11.31),女14人,男13人;第二组(3>病程≤6个月),24人,28耳,年龄18-53岁(36.15±11.45),女13人,男11人;第三组(6>病程≤12个月),28人,35耳,年龄22-51岁(36.68±8.44),女15人,男13人。对三组病人进行进行DPOAE检测,通过比较各组、各频点DPOAE反应幅值结果,应用SPSS统计学软件分析,初步评估病程长短与DPOAE反应幅值之间的关系。结果三组受试者在0.50-1.50kHz频点DPOAE反应幅值比较差异无统计学意义(分别为:P=0.141,P=0.226,P=0.898,P=0.138),而在2.00~8.00kHz频点DPOAE反应幅值比较差异有统计学意义(分别为:P=0.001,P=0.000,P=0.000,P=0.000,P=0.000)。结论DPOAE应用于听力正常的耳鸣患者中有着重要的临床意义,其能够在纯音听阈改变前发现患者早期耳蜗外毛细胞是否存在损伤,且随着病程的延长,其损伤可能越大,可作为耳蜗早期病变的一种客观的检测方法,让耳鸣患者早重视、早治疗,提高患者生活质量。Objective To report DPOAE amplitudes in patients with high-pitch tinnitus of different durations.Methods Patients with tinnitus and normal hearing were divided into three groups according to disease durations,i.e.≤3 months(n=27,31 ears,age=16-62 years[mean=38.57±11.31 years],female/male=14/13);3-6 months(n=24 peo-ple,28 ears,age=18-53 years[mean=36.15±11.45 years],female/male=13/11);and 6-12 months(n=28,35 ears,age=22-51 years[mean=36.68±8.44 years],female/male=15/13).DPOAE amplitudes at different frequencies were de-termined.The SPSS software was used to analyze the relationship between disease duration and DPOAE amplitude.Re-sults DPOAE amplitudes were not significantly different among the three groups at 0.5-1.5 kHz(P>0.05),but showed significant differences at 2-8 kHz(P<0.001).Conclusion DPOAE testing has important clinical significance in tinnitus patients with normal hearing and may detect early changes of outer hair cells before pure tone threshold changes.It can be used as an objective test for early lesions of the cochlea,allowing early intervention in patients with tinnitus to im-prove their quality of life.
