摘 要:目的:探讨用椎间孔镜(TESSYS)技术治疗翻转脱出型腰椎间盘突出症的临床效果。方法:从2016年6月至2017年6月期间在承德医学院附属医院脊柱外二科接受治疗的翻转脱出型腰椎间盘突出症患者中选取12例患者为研究对象。对这12例患者均采用TESSYS技术进行治疗。治疗结束后,观察这12例患者手术的情况和治疗的效果,对比接受治疗前后其腰椎疼痛症状及腰椎功能的改善情况。结果:这12例患者均顺利完成手术,其手术切口的平均长度为7mm,其术中平均的出血量为(4.1±1.2)ml,其平均的住院时间为9d,其治疗效果的优良率为83.3%。手术结束后、术后1个月、术后3个月、术后6个月及术后1年这12例患者的VAS评分和ODI评分均逐渐降低,与接受手术前相比,P<0.05。从手术结束后至术后1个月、术后3个月、术后6个月及术后1年,这12例患者腰椎JOA评分的改善率逐渐升高。结论:用TESSYS技术治疗翻转脱出型腰椎间盘突出症的效果确切、创伤性较小,可显著减轻患者腰椎疼痛的症状,改善其腰椎的功能。Objective:To investigate the clinical effect of TESSYS technique in the treatment of overturned prolapsed lumbar disc herniation.Methods:from June 2016 to June 2017,12 patients with overturned prolapsed lumbar disc herniation were selected as research objects.All 12 patients were treated with TESSYS technology.After the end of the treatment,the operation status and treatment effect of the 12 patients were observed,and the improvement of lumbar pain symptoms and lumbar function before and after the treatment were compared.Results:all the 12 patients successfully completed the operation,the average incision length was 7 mm,the average intraoperative blood loss was(4.1±1.2)ml,the average hospitalization time was 9 d,and the excellent and good rate of the treatment was 83.3%.VAS scores and ODI scores of the 12 patients decreased gradually after the operation,1 month after the operation,3 months after the operation,6 months after the operation and 1 year after the operation,P<0.05 compared with that before the operation.From the end of operation to 1 month after surgery,3 months after surgery,6 months after surgery and 1 year after surgery,the improvement rate of lumbar JOA score in these 12 patients gradually increased.Conclusion:TESSYS technique is effective and less invasive in the treatment of lumbar disc herniation caused by turnover and prolapse,which can significantly reduce the symptoms of lumbar pain and improve the function of the lumbar spine.
