急性闭合性跟腱断裂鞘膜修复的解剖学基础及临床应用

摘　　要：目的研究跟腱鞘膜的解剖形态及血供情况,为临床跟腱损伤修复鞘膜提供解剖学基础资料。方法利用10具乳胶灌注人体新鲜冰冻小腿标本进行跟腱区解剖,对跟腱的鞘膜解剖特征及其血管进行观察。结果跟腱外层筋膜鞘与内层滑膜鞘均有丰富的血供,且筋膜鞘与滑膜鞘之间、滑膜鞘与跟腱之间均存在间隙,可以完整剥离。结论跟腱筋膜鞘及滑膜鞘是跟腱的可修复的重要血供解剖结构,急性闭合跟腱断裂在临床手术时需保护及修复筋膜鞘及滑膜鞘。Objective To observe the anatomical morphology and blood supply of Achilles tendon sheath,and to provide anatomic data for sheath repair in acute closed Achilles tendon ruptures.Methods The anatomic features of the sheath and blood vessels of the Achilles tendon were observed by 10 fresh frozen lower leg specimens perfused with latex.Results The outer fascial sheath and inner synovial sheath of the Achilles tendon have abundant blood supply,and there are gaps between the fascial sheath and synovial sheath,and between the synovial sheath and the Achilles tendon,which can be completely stripped.Conclusion Both of the fascia sheath and the synovial sheath are important anatomical structures of blood supply for Achilles tendon.Acute closed Achilles tendon rupture requires the protection and repair of the fascial sheath and synovial sheath during clinical surgery.
