电视胸腔镜手术与开胸手术治疗外伤性血气胸的疗效比较

摘　　要：目的探讨电视胸腔镜手术与开胸手术治疗外伤性血气胸的疗效比较。方法选取2017年1月至2018年3月本院收治的72例外伤性血气胸患者,采用随机数字表达法分为对照组(36例)和研究组(36例)。研究组行电视胸腔镜手术,对照组行开胸手术。术后比较两组患者手术时间、住院时间、止痛药应用时间、胸腔引流时间、出血量及并发症情况。结果研究组患者手术时间(65.57±20.38)min、住院时间(10.37±2.67)d、术中出血量(538.46±32.18)ml、胸腔引流时间(3.22±1.28)d和止痛药应用时间(5.12±0.67)d,均低于对照组,比较差异有统计学意义(P<0.05)。研究术后并发症发生率(13.89%)明显低于对照组(33.33%),比较差异有统计学意义(P<0.001)。结论电视胸腔镜手术与开胸手术治疗外伤性血气胸均取得了一定的临床疗效,其中电视胸腔镜手术治疗外伤性血气胸的疗效和安全性更满意,符合外伤性血气胸手术的治疗规范,值得临床借鉴应用。Objective To compare the efficacy of video-assisted thoracoscopic surgery and thoracotomy in the treatment of traumatic pneumothorax.Methods 72 patients with traumatic hemopneumothorax admitted to our hospital from January 2017 to March 2018 were divided into control group(36 cases)and study group(36 cases)by random number expression method.The study group underwent video-assisted thoracoscopic surgery and the control group underwent thoracotomy.Postoperative operation time,length of stay,analgesic drug application time,chest drainage time,bleeding volume and complications were compared between the two groups.Results The operation time(65.57±20.38)min,hospitalization time(10.37±2.67)d,intraoperative bleeding volume(538.46±32.18)ml,thoracic drainage time(3.22±1.28)d and application time of analgesics(5.12±0.67)d in the study group were lower than those in the control group,and there were statistical differences(P<0.05).The incidence of postoperative complications(13.89%)in the study group was significantly lower than that in the control group(33.33%)with statistical difference(P<0.001).Conclusion Video-assisted thoracoscopic surgery and thoracotomy for the treatment of traumatic hemothorax have achieved a certain clinical effect,of which video-assisted thoracoscopic surgery for traumatic hemothorax is more effective and safe for the treatment of traumatic hemothorax,and it is worthy of clinical application.
