2016年至2018年本院围手术期质子泵抑制剂使用情况分析及合理性评价

摘　　要：目的分析本院2016年至2018年围手术期患者质子泵抑制剂(PPI)的使用状况和合理性评价。方法利用本院医院信息系统(HIS),随机选取2016年1月至2018年6月间出院病例600例,回顾性调查患者临床资料、手术状况、SU相关风险因素、使用PPI状况、医疗费用,对上述结果进行统计分析。结果患者手术类型中胃肠手术、骨科手术及乳腺手术比例最多,分别为25.00%、21.83%、17.17%;在PPI使用方面,没有联合用药者,静脉滴注泮托拉唑比例最高,为43.16%,其次为拉索拉唑,为29.00%;各类型手术患者中均存在不合理使用PPI,其中胃肠手术、乳腺手术、肝胆手术不合理率最高,分别为94.67%、90.29%、89.19%;在患者PPI不合理使用分类中,无指征用药比例最高,为38.99%,其次为用法用量不适宜,为30.51%。结论临床围手术期患者不合理使用PPI情况比较突出,还需进一步规范管理PPI的使用。Objective To analyze the use and rationality of proton pump inhibitor(PPI)in perioperative patients from 2016 to 2018.Methods Using hospital information system(HIS),we randomly selected 600 cases of patients discharged from January 2016 to June 2018.The clinical data,surgical status,SU related risk factors,use of PPI status,medical expenses were retrospectively investigated,statistical analysis of the above results.Results The proportion of gastrointestinal surgery,orthopedic surgery and breast surgery was the highest in the patients'surgical types,which were 25.00%,21.83%,and 17.17%,respectively.In the use of PPI,there was no combination of patients,and the proportion of intravenous infusion of pantoprazole was the highest 43.16%,followed by lasoprazole 29.00%;there were unreasonable use of PPI in all types of surgical patients,of which gastrointestinal surgery,breast surgery,hepatobiliary surgery had the highest unreasonable rate 94.67%,90.29%,89.19%.In the classification of patients with unreasonable use of PPI,the proportion of non-indications was the highest,being 38.99%,followed by the inappropriate dosage,which was 30.51%.Conclusion The unreasonable use of PPI in patients with perioperative period is more prominent,and further regulation of the use of PPI is needed.
