多囊卵巢综合征不孕患者的影响因素分析与ATR助孕结局的研究

摘　　要：目的:探讨多囊卵巢综合征(PCOS)不孕患者的影响因素以及分析ATR助孕后的妊娠率。方法:选取2017年1月-2018年12月太原市中心医院生殖中心收治的984例不孕患者作为研究对象,分为PCOS组与非PCOS组。收集患者的一般资料,采用卡方检验与Logistic回归进行单因素、多因素分析。比较PCOS组与非PCOS组ATR助孕后的临床妊娠率。结果:单因素分析显示:PCOS组与非PCOS组在年龄、不孕年限、妊娠史差异无统计学意义(P>0.05),而在肥胖、有PCOS家族史、有妇科慢性炎症史、有甲状腺疾病史、有糖尿病/血糖高史差异有统计学意义(P<0.05)。Logistic回归多因素分析显示:调整其他因素后,肥胖、PCOS家族史、甲状腺疾病史为PCOS不孕患者的独立影响因素(P<0.05)。接受ATR助孕后PCOS组的妊娠率(20.25%)低于非PCOS组的妊娠率(42.00%)。结论:肥胖、PCOS家族史、甲状腺疾病史为PCOS致不孕的独立影响因素,且接受ATR助孕妊娠率低于其他因素不孕患者。Objective:To investigate the influencing factors of infertility patients with polycystic ovary syndrome and to analyze the pregnancy rate after receiving ATR.Methods:A total of 984 infertile patients admitted to the Reproductive Center of Taiyuan Central Hospital from January 2017 to December 2018 were enrolled in the PCOS and non-PCOS groups.Data were collected from patients and single factor and multivariate analysis were performed using chi-square test and logistic regression.The clinical pregnancy rate after ATR assisted pregnancy was compared between the PCOS group and the non-PCOS group.Resuilts:Univariate analysis showed that there was no significant difference in age,infertility,and pregnancy history between PCOS group and non-PCOS group(P>0.05),but there was a statistically significant difference in obesity,family history of PCOS,history of chronic inflammation of gynecology,and thyroid gland and the history of diabetes/blood sugar(P<0.05).Logistic regression multivariate analysis showed that after adjusting for other factors,obesity,family history of PCOS,and history of thyroid disease were independent factors influencing PCOS infertility patients(P<0.05).The pregnancy rate(20.25%)in the PCOS group after ATR-assisted pregnancy was lower than that in the non-PCOS group(42.00%).Conclusion:Obesity,family history of PCOS,and history of thyroid disease are independent influencing factors of infertility caused by PCOS,and the pregnancy rate of PCOS group is lower than that of non-PCOS group with therapy of ATR.
