摘  要:目的探讨超声引导下体外冲击波疗法(ESWT)治疗过肩项目运动员肩峰下撞击综合征(SIS)的临床疗效。方法22名SIS过肩项目运动员,随机分成各为11名的超声引导下体外冲击波治疗组(实验组)和体外冲击波治疗组(对照组),并分别于治疗前、治疗4周后,采用视觉模拟评分(VAS)、肩关节活动度(ROM)、调整型Constant-Murley肩关节功能评分(CMS)表来评估运动员肩关节疼痛和肩关节运动功能障碍的改善情况。结果治疗4周后,两组研究对象VAS评分、肩关节活动度及CMS评分较治疗前均有明显改善且优于对照组(p<0.05)。结论本研究显示超声定位法可准确找到SIS运动员的具体病变部位,定位精准后再进行体外冲击波治疗后,能有效提高SIS的临床疗效,并且是安全的,值得在临床上推广应用,但其机制仍需进一步研究。Objective To explore the clinical effect of ultrasound-guided extracorporeal shockwave therapy (ESWT) on overhead athletes with subacromial impingement syndrome(SIS). Methods Twenty-two athletes with SIS were randomly divided into ultrasound-guided ESWT group(experimental group) and control group. Participants‘visual analog scale(VAS), shoulder’s range of motion (ROM), adjusted Constant-Murley score(CMS) were measured before and after the test to assess shoulder pain and shoulder joint function. Results After 4 weeks ESWT treatment, the comparison between two groups showed that the score of VAS, ROM and CMS were significantly improved, and the experimental group were significantly higher than those in the control group(p<0.05). Conclusion This study shows that the ultrasound-guided localization method can accurately find the specific lesions of SIS athletes. After precise localization and extracorporeal shockwave therapy, it can effectively improve the clinical efficacy of SIS, which is worth popularizing and applying in clinic, but its mechanism still needs further study.
