速干手消毒剂与六步洗手法对比临床应用的系统评价

摘　　要：目的评价速干手消毒剂与六步洗手法对比临床应用的效果。方法检索主要中英文数据库从建库至2016年12月31日速干手消毒剂与六步洗手法对比临床应用随机对照研究。应用Cochrane评价员手册及Jadad评分法进行偏倚风险、质量评价。应用RevMan 5.3软件进行Meta分析。结果符合纳入标准的文献共6篇。Meta分析结果显示,速干手消毒剂与六步洗手法对比临床应用,依从性比值比(odds ratio,OR)为30.26,95%置信区间[5.46,167.64],差异有统计学意义(P<0.001);洗手时间均数差为-132.72,95%置信区间[-214.58,-50.85],差异有统计学意义(P=0.001);洗手合格率、洗手后细菌数方面无明显差异(P>0.05)。结论速干手消毒剂与六步洗手法对比能有效提高医务人员依从性,缩短洗手时间,在洗手合格率、洗手后细菌数方面无明显差异,值得临床应用。但仍需要更多大样本、高质量的随机试验进一步验证。Objective To evaluate the effect of clinical application of quick-drying hand disinfectant versus six step hand washing method.Methods Main databases were electronically searched from the inception of each database up to 31 December 2016.Randomized clinical trials(RCTs)of quick-drying hand disinfectant versus six step hand washing method for the clinical application were included.The Cochrane risk of bias tool was used to assess the risk of bias and the Jadad rating scale was used to assess the quality of included studies.The Cochrane Collaboration's RevMan5.3 software was used for meta-analysis.Results Six studies met the inclusion criteria.The meta-analysis results showed that the comparison of quick-drying hand disinfectant and six step hand washing method was applied in clinics,the compliance rate odds ratio was 30.26,with 95%confidence interval[5.46,167.64],and the difference was statistically significant(P<0.001).The washing time mean difference was-132.72,with 95%confidence interval[-214.58,-50.85],and the difference was statistically significant(P=0.001).There is no significant difference in the qualified rate of hand washing and the bacteria number after washing hands(P>0.05).Conclusion Compared to six step hand washing method,quick-drying hand disinfectant for the clinical application can effectively improve the medical staff hand washing compliance,shorten the washing time and has significant effect.
