经皮椎弓根钉置入内固定术治疗胸腰椎骨折26例

摘　　要：目的探讨经皮椎弓根钉置入内固定治疗胸腰椎骨折的疗效。方法选取2011年6月至2016年7月陕西省陇县中医医院胸腰椎骨折病人52例,采用随机数字表法分为微创组和对照组,各26例。对照组采用传统开放椎弓根钉置入内固定治疗,微创组采用经皮椎弓根钉置入内固定治疗。比较两组治疗效果,平均引流量、术中收集血量、术后机体疼痛积分和平均住院时间,干预前后病人椎体高度、椎间隙高度、椎体COBB角。结果两组治疗效果比较,对照组总有效率为92.31%,微创组为96.15%,差异无统计学意义(P>0.05);微创组平均引流量(43.51±1.41)mL、术中收集血量(25.51±2.61)mL、术后机体疼痛积分(3.62±1.21)分和平均住院时间(7.51±1.24)d低于对照组[(154.26±2.77)mL、(57.26±3.57)mL、(5.61±2.59)分、(9.26±2.17)d](P<0.05);干预前后两组椎体高度、椎间隙高度、椎体COBB角相近,差异无统计学意义(P>0.05)。结论椎弓根钉置入内固定治疗胸腰椎骨折疗效确切,在保证椎体高度、椎间隙高度、椎体COBB角恢复正常的前提下,经皮椎弓根钉置入内固定术可改善胸腰椎骨折病人椎体功能,减轻机体疼痛,减少出血和引流量,缩短住院时间。Objective To analyze the effect of invasive pedicle screw fixation in the treatment of thoracolumbar fractures.Meth?ods Fifty?two patients with thoracolumbar fractures in Qixian Chinese Medicine Hospital of Shaanxi Province from June 2011 to July 2016 were selected and assigned into minimally invasive group and control group according to the random number table,with 26 cases in each group.The control group was treated with traditional open pedicle screw fixation,and the minimally invasive group was treated with minimally invasive pedicle screw fixation.The treatment effects of thoracolumbar fractures were compared between the two groups.The average volume of flow,volume of blood collected,postoperative pain score and average length of stay were mea?sured.The height of vertebral body,the height of intervertebral space and the angle of COBB of vertebral body were measured before and after intervention.Results The treatment effective of the control group and the minimally invasive group were with no sig?nificant difference(92.31% vs.96.15%,P<0.05).The average drainage,intraoperative blood collection,postoperative pain of bodypoints and the average hospitalization time of the minimally invasive group was lower than those of the control group[(43.51±1.41)mL vs.(154.26±2.77)mL,(25.51±2.61)mL vs.(57.26±3.57)mL,(3.62±1.21)points vs.(5.61±2.59)points,and(7.51±1.24)d vs.(9.26±2.17)d,P<0.05].There were no significant differences in vertebral height,intervertebral height,vertebral COBBangle between the two groups(P>0.05).Conclusion The pedicle screw fixation for thoracolumbar fractures is effective,whichcan promote the vertebral height,intervertebral height,vertebral COBB angle normal vertebral body function,relieve pain,reducebleeding and drainage,and shorten the hospitalization time.
