助产护理模式对高龄产妇分娩方式及产程的影响

摘　　要：目的:观察助产护理模式对高龄产妇分娩方式及产程的影响。方法:选取84例高龄产妇作为研究对象,依据随机数字表法将其分为对照组和研究组各42例,对照组实施常规护理,研究组在对照组的基础上实施助产护理,比较两组分娩方式、各产程时间、产后出血情况和新生儿阿氏评分。结果:研究组自然分娩率明显高于对照组,剖宫产率明显低于对照组,差异均有统计学意义(P<0.05);研究组第一、二产程和总产程明显短于对照组,差异有统计学意义(P<0.05);研究组产后出血率明显低于对照组,新生儿阿氏评分明显高于对照组,差异均有统计学意义(P<0.05)。结论:在常规护理的基础上实施助产护理模式,可明显提高高龄产妇的自然分娩率,有效缩短产程,减少产后出血,且新生儿阿氏评分明显优于常规护理。Objective: To study effects of midwifery nursing mode on delivery mode and birth process of elderly parturient women.Methods: 84 elderly parturient women were selected as the study subjects. They were divided into control group(n=42) and study group(n=42)according to the random number table method. The control group received routine nursing care, while the study group was implemented the midwifery nursing on the basis of that of the control group. The delivery mode, duration of each labor stage, postpartum hemorrhage, and neonatal Apgar score were compared between the two groups. Results: The spontaneous delivery rate of the study group was significantly higher than that of the control group;the cesarean section rate was significantly lower than that of the control group;and the differences were statistically significant(P<0.05). The time of the first and the second stages of labor as well as the total labor duration of the study group were significantly shorter than those of the control group, and the differences were statistically significant(P<0.05). The postpartum hemorrhage rate of the study group was significantly lower than that of the control group;the newborn Apgar score was significantly higher than that of the control group;and the differences were statistically significant(P<0.05). Conclusions: The midwifery nursing mode on the basis of the routine nursing care can significantly improve the spontaneous delivery rate of the elderly parturient women, effectively shorten the labor process and reduce the postpartum hemorrhage. Moreover, the newborn Apgar score is significantly better than that of the conventional nursing care.
