四肢手术摆位固定冲洗盘在手显微外科手术中的应用

摘　　要：目的设计一种四肢手术摆位固定冲洗盘,并观察其在手显微外科手术中的临床使用效果。方法将2016年1月至2017年12月在南方医科大学附属小榄医院行单指断指再植手术患者80例,按手术先后顺序分为对照组和观察组各40例,对照组采用助手扶持固定患肢,观察组采用四肢手术摆位固定冲洗盘固定患肢及接收与排放冲洗液。比较两组患者的手术时间、手术台面和手术间地面污染情况及血管一次性吻合成功率。结果观察组患者的手术时间为(125.08±6.56)min,明显低于对照组的(142.20±5.48)min,手术台面和手术间地面污染发生率分别为45.0%、0,明显少于对照组的95.0%、97.5%,血管一次性吻合成功率为97.5%,明显高于对照组的80.0%,差异均有统计学意义(P<0.05)。结论应用四肢手术摆位固定冲洗盘不仅有效提高手术效率,减轻手术人员的工作强度,而且可以减少手术台面及手术间地面污染的发生率,实用性强,值得推广应用。Objective To design a fixed flushing disk for limb surgery and observe its clinical effect in hand microsurgery.Methods A total of 80 cases of replantation of severed finger,who admitted to Xiaolan Hospital Affiliated to Southern Medical University from January 2016 to December 2017,were selected and divided into the control group and observation group according to the order of operation,with 40 cases in each group.Assistant support to fix the affected limbs was used to the control group,while limb surgical placement to fix the affected limbs,receive and drain the flushing fluid was used to the observation group.The operation time,surface contamination and the success rate of one-off anastomosis between the two groups were compared.Results The operation time of the observation group was(125.08±6.56)minutes,which was significantly shorter than(142.20±5.48)minutes of the control group;the incidence of surface contamination in operating table and operating room was 45.0%and 0 respectively,which was significantly lower than corresponding 95.0%and 97.5%in control group(all P<0.05).The success rate of one-off anastomosis was 97.5%,which was significantly higher than 80.0%of the control group(P<0.05).Conclusion The application of fixed irrigating disc in extremity surgery not only effectively improves the efficiency of operation and reduces the intensity of operation personnel,but also reduces the incidence of surface contamination of operation table and operation room.It is practical and worthy of popularization and application.
