摘 要:目的研究甘精胰岛素联合二甲双胍对于2型糖尿病患者胰岛功能的治疗效果,及其对患者血清炎性因子水平的影响。方法2015年12月至2017年2月收治的60例确诊为2型糖尿病的患者,按照随机分配的原则分为观察组和对照组,每组30例。在饮食、运动的基础上,观察组使用甘精胰岛素皮下注射联合二甲双胍治疗,对照组仅给予患者二甲双胍进行治疗,疗程3个月,比较2组患者治疗前后血糖控制情况、胰岛素抵抗指数(Homa IR)、超敏CRP(hs-CRP)、肿瘤坏死因子(TNF-α)、白介素6(IL-6)。结果2组治疗后空腹血糖、餐后2 h血糖、Homa IR、超敏CRP、TNF-α、IL-6结果较治疗前均有明显下降,治疗组较对照组更为明显(P<0.05)。结论甘精胰岛素联合二甲双胍除降糖效果显著外,可有效改善2型糖尿病患者血清炎性因子水平,减轻胰岛素抵抗,改善胰岛功能。Objective To investigate the therapeutic effects of insulin glargine combined with dimethyldiguanide on type 2 diabetes mellitus,and to observe its effects on islet function and inflammatory factors of patients.Methods Sixty patients who were diagnosed as type 2 diabetes mellitus in our hospital from December 2015 to February 2017 were randomly divided into observation group(n=30)and control group(n=30).Based on diet and exercise remedy,the patients in observation group were treated by hypodermic injection of insulin glargine and oral dimethyldiguanide,however,the patients in control group were treated by dimethyldiguanide only,with a treatment course of 3 months for both groups.The levels of blood glucose(BLG),insulin resistance index(HomaIR),hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor(TNF-α)and interleukin 6(IL-6)were observed and compared between the two grousp.Results After treatment the levels of fasting blood-glucose,2-hour postprandial blood glucose,HomaIR,hs-CRP,TNF-αand IL-6 were decreased in both groups,moreover,which in observation group were significantly lower than those in control group(P<0.05).Conclusion The insulin glargine combined with dimethyldiguanide has obvious efffects of reducing blood glucose,which can effectively improve the serum levels of inflammatory factors,relieve insulin resistance and improve pancreas islet function of patients with type 2 diabetes mellitus.
