集束化健康教育在腹腔镜胰十二指肠切除术中的应用

摘　　要：目的研究集束化健康教育在腹腔镜胰十二指肠切除患者康复中的应用效果。方法80例行腹腔镜胰十二指肠切除的患者,按随机数字表法分为对照组(n=40)和观察组(n=40)。对照组采用常规健康教育方法,观察组采用加速康复理念下的集束化健康教育方法。评价2组患者术后留置胃管时间、首次排气时间、恢复进食时间及相关并发症发生情况。结果观察组患者留置胃管时间、首次排气时间、恢复进食时间及首次下床时间均短于对照组(P<0.05);观察组术后相关并发症发生率为5.0%明显低于对照组的22.5%,差异有统计学意义(P<0.05);观察组健康教育满意度为97.5%明显高于对照组的72.5%,差异有统计学意义(P<0.05)。结论加速康复理念的集束化健康教育可提高患者治疗护理依从性,可有效缩短腹腔镜胰十二指肠切除患者留置胃管时间,促进排气,早期进食,降低相关并发症发生率,提高健康教育满意度,对术后患者快速康复起到至关重要的作用。Objective To investigate the application effect of cluster health education in the rehabilitation of patients undergoing laparoscopic pancreaticoduodenectomy.Methods Eighty patients who underwent laparoscopic pancreaticoduodenectomy were divided into control group(n=40)and observation group(n=40)according to the random number table method.The patients in control were treated by conventional health education method,however,the patients in observation group were treated by cluster health education method based on the concept of accelerated rehabilitation.The time of indwelling gastric tube,the first exhaust time,the time of resuming eating and the related complications were observed and compared between the two groups.Results The indwelling time of gastric tube,the first exhaust time,the recovery time of feeding and the first time of getting out of bed in observation group were significantly shorter than those in control group(P<0.05).The incidence rate of postoperative complications in observation group was 5%,which was significantly lower than that(22.5%)in control group(P<0.05).The satisfaction degree of health education in observation group was 97.5%,which was significantly higher than that(72.5%)in control group(P<0.05).Conclusion The cluster health education based on the concept of accelerated rehabilitation can improve the treatment and nursing compliance of patients,effectively shorten the time of indwelling gastric tube after laparoscopic pancreaticoduodectomy,promote exhaust and early feeding,reduce the incidence rate of related complications,and improve the satisfaction degree of health education,which plays a crucial role in the rapid recovery of patients after operation.
