健康教练技术在老年2型糖尿病病人延续性护理中的应用研究

摘　　要：[目的]探讨健康教练技术运用于老年2型糖尿病病人出院后的延续性护理效果。[方法]将在我院内分泌科就诊的108例老年2型糖尿病病人随机分为对照组(53例)和试验组(55例)。出院后,对照组采用传统常规延续性护理,试验组采取健康教练技术进行延续性护理,比较干预前1 d及干预后6个月两组的血糖变化情况、自我管理能力得分变化情况及试验组应用健康教练技术的效果评价。[结果]干预前1 d两组老年病人的空腹血糖、糖化血红蛋白、自我管理能力比较,差异均无统计学意义(P>0.05);干预6个月后,试验组空腹血糖、糖化血红蛋白显著低于对照组(P<0.05),试验组自我管理能力各维度(饮食方面、运动方面、血糖监测、足部护理、用药方面)及总分显著优于对照组(P<0.05)。试验组对健康教练技术的应用效果评价得分率均在90%以上。[结论]健康教练技术有利于控制老年2型糖尿病病人血糖,提高老年2型糖尿病病人的自我管理能力,达到良好的延续性护理效果。Objective:To explore the application effect of health coaching technique in continuous nursing of elderly patients with type 2 diabetes after discharge.Methods:A total of 108 elderly patients with type 2 diabetes admitted to the department of endocrinology in our hospital were randomly divided into the control group(53 cases)and the experimental group(55 cases).After discharge from the hospital,the control group received traditional routine continuous nursing.The experimental group received the health coaching technique for conducting continuous nursing.The changes of blood glucose and self-management ability scores were compared between the two groups before the intervention and six months after the intervention.The application effect of health coaching technique in the experimental group was evaluated.Results:There were no significant differences in fasting blood glucose(FPG)and glycosylated hemoglobin(HbAlc)and self?maragement ability between the two groups before the intervention(P>0.05).The FPG and HbAlc in the 6-month intervention group were significantly lower than those in the control group(P<0.05).The self-management ability of the experimental group(dietary,exercise,blood glucose monitoring,foot care,medication)and total score were significantly higher than those of the control group(P<0.05).The application effect evaluation of experimental group on health coaching technology was above 90%.Conclusions:The health coaching technique was beneficial to control the blood sugar of elderly patients with type 2 diabetic,to improve their selfmanagement ability,and to achieve a good continuous nursing effect.
