摘 要:目的:探讨益肾降浊冲剂减缓慢性肾功能衰竭(CRF)的恶化进程是否通过减少活性氧、并抑制线粒体介导的肾小管细胞凋亡实现的。方法:采用5/6肾切除大鼠CRF模型,随机分为模型组、大黄组及益肾降浊冲剂大、小剂量组,观察肾脏病理改变、测定各组尿素氮(BUN)、肌酐(Scr)、总胆固醇(TC)、甘油三酯(TG)、超氧化物歧化酶(SOD)、丙二醛(MDA)的表达水平,以及利用免疫组化观察肾脏组织的Caspase-3和Caspase-9的表达情况。结果:大黄素组和益肾降浊冲剂大剂量组BUN、Scr、TG、MDA的表达水平和Caspase-3与Caspase-9的IOD值均显著低于模型组(P<0.01),SOD显著高于模型组(P<0.01);HE染色下观察肾脏细胞,模型组的病变最重;益肾降浊冲剂大剂量组的病变最轻;大鼠肾组织自由基的水平与Scr、BUN呈显著正相关,并与肾脏病理、肾小管上皮细胞Caspase-3和Caspase-9的表达量相一致。结论:益肾降浊冲剂可能通过减少和清除产生的活性氧保护肾小管线粒体,从而最大限度减少肾小管的细胞凋亡,起到治疗CRF的作用。Objective: To explore whether Yishen Jiangzhuo Granules slows the progression of chronic renal failure(CRF)by reducing active oxygen and inhibiting mitochondria-mediated apoptosis of renal tubular epithelial cells. Methods: Rats were given 5/6 subtotal nephrectomies to establish the model of CRF. The model of CRF rats were randomly divided into 4 groups: the model control group, the Chinese Rhubarb group, the large and small dose of Yishen Jiangzhuo Granules group. The expressions of BUN, Scr, TC, TG, SOD and MDA were measured in all rats. Immunohistochemistry was performed to detect the expressions of Caspase-3 and Caspase-9. Results: Compared with the normal control group, the BUN, Scr, TG, MDA levels and IOD of Caspase-3 and Caspase-9 of the Chinese Rhubarb group and the large dose of Yishen Jiangzhuo Granules group were significantly lower(P<0.01), and SOD was significantly increased(P<0.01). The variations of HE showed the damage of the model control group is the most serious, that of the large dose of Yishen Jiangzhuo Granules group is the slightest of all. A significantly positive-dependent relationship was observed between the expressions of free radicals of nephridial tissues and Scr, BUN. And the expressions of free radicals of nephridial tissues are the same as pathological morphology of nephridial tissues and IOD of Caspase-3 and Caspase-9 of renal tubular epithelial cell. Conclusion: Yishen Jiangzhuo Granules can reduce the apoptosis of renal tubules, possibly through reduced and cleared free radicals and protected the mitochondria of renal mitochondria, playing a role in treating chronic renal failure.
