期待治疗未足月胎膜早破孕妇残余羊水量对母婴结局的影响

摘　　要：目的研究采用期待疗法治疗未足月胎膜早破(preterm premature rupture of membrane,PPROM)孕妇的残余羊水量对分娩方式以及并发症的影响。方法100例31周~33周+6天的PPROM孕妇中羊水指数(AFI)<50 mm的羊水过少孕妇29例(甲组),50 mm≤AFI<80 mm 34例(乙组),80 mm≤AFI<250 mm37例(丙组),均根据孕妇本人及其家属的意愿采用期待疗法治疗;统计各组孕妇分娩情况以及母婴围产期并发症发生情况。结果丙组孕妇分娩间隔时间大于甲、乙两组,甲组剖宫产率(51.72%)高于乙、丙两组,差异均有统计学意义(P<0.05);甲组分娩并发症的总发生率(37.93%)、围生儿不良结局的发生率(27.58%)均高于乙、丙两组,差异有统计学意义(P<0.05)。结论羊水过少可能引发孕妇剖宫产率增加以及母婴围产期并发症发生率升高。Objective To evaluate the effect of expectant therapy for residual amniotic fluid volume in women with preterm premature rupture of membranes(PPROM)on the occurrence of complications and the choice of delivery mode.Methods Firstly,100 patients with PPROM(before 31 to 33 weeks+6 days)were divided into three groups,namely,group A(n=29,amniotic fluid index(AFI)<50 mm),group B(n=34,50 mm≤AFI<80 mm),and group C(n=37,80 mm≤AFI<250 mm).All groups were subjected to expectant therapy as agreed by the subjects and their families.The incidence of perinatal complications and parturient complications were then documented and analyzed.Results The interval of delivery in group C was longer than that either in group A or in group B.The cesarean section rate(51.72%)of group A was higher than that of either group B or group C.The difference was statistically significant(P<0.05).Both the rate of incidence of childbirth complications(37.93%)and the rate of incidence of adverse perinatal outcomes(27.58%)of group A were higher than those of either group B or group C.The differences were statistically significant(P<0.05).Conclusions Oligohydramnios in the wake of PPROM may lead to greater rate of cesarean section and perinatal complications.
