血糖和血脂对慢性阻塞性肺疾病急性加重期患者气流受限程度的影响分析

摘　　要：目的探讨不同气流受限程度的AECOPD患者血糖、血脂的变化特点及对预后影响。方法收集2017年06月至2018年09月本医院收治的AECOPD患者73例(其中轻中度组36例,重度、极重度组37例),及同期39例健康体检者作为对照组。检测各组的空腹血糖(FBG)、血脂水平,并进行分析比较。结果AECOPD患者组FBG较对照组升高,重度、极重度组FBG高于轻中度组,差异均有统计学意义(P<0.05);轻中度组总胆固醇(TC)较重度、极重度组和对照组升高(P<0.05);重度、极重度组低密度脂蛋白(LDL-C)水平较对照组和轻中度组升高(P<0.05);重度、极重度组FBG与FEV 1%pred呈负相关;logistic回归分析显示,FBG和TC是AECOPD气流受限严重程度的影响因素。结论AECOPD患者存在血糖、血脂紊乱且与气流受限严重程度相关,不利于疾病的治疗及预后。Objective To investigate the changes of blood glucose and blood lipid in AECOPD patients with different severity of airflow limitation and their influence on prognosis.Methods 73 patients with AECOPD from June 2017 to September 2018 were recruited,with 36 cases in the mild and moderate group and 37 cases in the severe and extremely severe group.During the same period,39 healthy people were taken as the control group.The fasting blood glucose(FBG)and blood lipid levels of each group were analyzed and compared.Results The level of FBG in the AECOPD group was higher than in the control group(P<0.05),and it was higher in the severe and extremely severe group than in the mild and moderate group(P<0.05).The level of TC in the mild and moderate group was higher than in other two groups.The level of LDL-C in the severe and extremely severe group was higher than in the control group and the mild and moderate group(P<0.05).FBG in the severe and extremely severe group was negatively correlated with FEV 1%pred.Logistic regression analysis showed that FBG and TC were the influence factors for the severity of airflow limitation in AECOPD patients.Conclusion AECOPD patients have blood glucose and blood lipid disorders,which may be related to the severity of airflow limitation in patients with AECOPD and is bad for the treatment and prognosis of the disease.
