摘　　要：目的分析以SOAP门诊药历为主体的药学服务对改善患者用药依从性的影响。方法为患者建立SOAP门诊药历,结合患者用药习惯,给予患者以及患者家属精准用药宣教和指导,与医师共同优化治疗方案,给予个体化药学监护等,以患者的用药依从性为指标评价干预结果。结果干预24周后,患者用药依从性有明显提高;平均药费与人均药品数降低,与第1周结果比较,差异有统计学意义(P<0.05)。结论通过SOAP门诊药历干预模式能有效提高患者用药依从性。Objective To analyze the effect of pharmaceutical care with SOAP outpatient medical history as the main body on improving drug compliance of patients.Methods The outpatient medication record by SOAP mode was established.Combining with patients medication habits,precise medication guidance,optimal treatment plans and individual pharmaceutical care,the intervention results were evaluated with patient medication compliance as an index.Results Compared with the results of the first week,patient medication compliance was significantly improved after 24 weeks of intervention.Conclusion Intervention model with SOAP medication record can effectively improve patient drug compliance.
