摘  要:目的探讨椎动脉间段反向血流频谱在判断颈部血管病变部位中的应用价值。方法83例明确诊断为椎动脉病变的患者,均行颈动脉彩色多普勒超声检查。所有患者椎动脉椎间段血流频谱呈反向。根据血管病变部位,将患者分为锁骨下动脉狭窄组61例和椎动脉起始段闭塞组22例。观察患者椎动脉椎间段血流及频谱图像,比较两组患侧椎动脉血流速度及内径、对侧椎动脉血流频谱参数。结果锁骨下动脉狭窄组的患侧椎动脉椎间段血流频谱形态在收缩期及舒张期均表现为反向血流信号,椎动脉起始段闭塞组在收缩期表现为反向的低速血流信号,而在舒张期无血流信号。椎动脉起始段闭塞组患侧椎动脉收缩期及舒张期流速均慢于锁骨下动脉狭窄组(均P<0.05),两组对侧椎动脉血流频谱参数差异均无统计学意义(均P>0.05)。结论椎动脉椎间段反向血流频谱能够为快速寻找颈部血管病变部位提供重要线索。Objective To investigate the application value of reverse-flow spectrum in the intervertebral segment of vertebral artery for assessing the lesion of cervical vascular disease.Methods Eighty-three patients definitely diagnosed with vertebral artery disease underwent carotid color Doppler ultrasound examination.All cases presented reverse-flow spectrum in the intervertebral segment of vertebral artery.The patients were divided into subclavian artery stenosis group(n=61)and occlusive initial segment of vertebral artery group(n=22)according to the lesion of vascular disease.The blood flow and flow spectrum images of the intervertebral segment of vertebral artery were observed in the patients,besides,the blood velocity and internal diameter of the affected vertebral artery,and the flow spectrum parameters of the contralateral vertebral artery were compared between the two groups.Results The flow spectrum in the intervertebral segment of the affected vertebral artery was presented as reverse flow signals during systole and diastole phases in the subclavian artery stenosis group,and as reverse and low-velocity signals only during systole phase in the occlusive initial segment of vertebral artery group without flow signal during diastole phase.The occlusive initial segment of vertebral artery group had lower flow velocities of the affected vertebral artery during systole and diastole phases compared with the subclavian artery stenosis group(all P<0.05),moreover,there were no statistically significant differences in flow spectrum parameters of the contralateral vertebral artery between the two groups(all P>0.05).Conclusion Reverse-flow spectrum of the intervertebral segment of vertebral artery can provide important clues for quickly locating the lesions of cervical vascular disease.
