疏肝活血汤联合祛白酊治疗白癜风的疗效及其机制研究

摘　　要：目的:观察疏肝活血汤联合祛白酊治疗白癜风的疗效及对皮肤组织液内皮素1(ET-1)、神经肽(NPY)和血清铜、锌水平的影响。方法:将白癜风患者122例按照随机数字表法分为观察组和对照组各61例,2组均给予健康教育,在此基础上对照组给予祛白酊治疗,观察组在对照组基础上联合疏肝活血汤治疗。观察2组患者中医证候积分、皮损色素积分、皮损面积、临床疗效,并检测患者皮肤组织液中ET-1、NPY水平和血清铜、锌含量。结果:治疗后2组患者皮损色素积分高于治疗前(P<0.05),且观察组高于对照组(P<0.05),2组患者中医证候积分、皮损面积低于治疗前(P<0.05),且观察组低于对照组(P<0.05);治疗后2组患者ET-1、水平高于治疗前(P<0.01),且观察组高于对照组(P<0.05),而NPY水平低于治疗前(P<0.05),且观察组低于对照组(P<0.01);治疗后2组患者血清铜、锌含量高于治疗前(P<0.05),且观察组高于对照组(P<0.05);观察组总有效率为91.8%,高于对照组的77.0%(P<0.05)。结论:疏肝活血汤联合祛白酊可有效改善白癜风患者皮肤组织液ET-1、NPY水平,提高血清铜、锌含量和临床疗效。Objective:To observe the effects of liver-soothing blood-activating decoction combined with leucoderma-removing tincture in the treatment for leucoderma and its influence on endothelin-1(ET-1)and neuropeptide Y(NPY)in skin tissue and the contents of copper and zinc in the serum.Methods:All 122 patients were allocated to the observation group and the control group according to random number table method,61 cases each group,both groups accepted health education,the control group was given leucoderma-removing tincture,and the observation group liver-soothing blood-activating decoction on the basis.To observe TCM syndrome scores,skin pigment scores,skin lesion area,clinical effects between both groups,and measure the levels of ET-1 and NPY,and the contents of copper and zinc in the serum.Results:Skin pigment scores of both groups after treating were higher than these before treating(P<0.05),and the observation group was higher than the control group(P<0.05),TCM syndrome scores and skin lesion area of both groups were lower than these before treating(P<0.05),and the observation group was lower than the control group(P<0.05);the levels of ET-1 and NPY of both groups after treating were higher than before treating(P<0.01),and the observation group was higherthan the control group(P<0.05),the level of NPYwas lower than before treating(P<0.05),and the observation group was lower than the control group(P<0.01);the contents of zinc and copper in both groups after treating were higher than before treating(P<0.05);and the observation group was higher than the control group(P<0.05);Total effective rate of the observation group was 91.8%,higher than 77.0%of the control group(P<0.05).Conclusion:Liver-soothing blood-activating decoction combined with leucoderma-removing tincture in the treatment for leucoderma could notably improve the levels of ET-1 and NPY,raise the contents of zinc and copper and clinical effects.
