血清HCG、E2、P、CA125水平对早期异位妊娠与先兆流产诊断价值

摘　　要：目的:探讨早期妊娠血清人绒毛膜促性腺激素(HCG)、雌二醇(E2)、孕酮(P)及血清癌相关糖抗原125(CA125)水平在异位妊娠与先兆流产鉴别诊断中的应用价值。方法:回顾性收集本院妇产科接诊的早期先兆流产48例(先兆组)、异位妊娠32例(异位组)、正常妊娠孕妇80例(正常组)。检测血清β-hCG、E2、P、CA125水平,分析诊断效能。结果:正常组的P、E2、β-hCG水平最高,CA125水平最低,异位组的P、E2、β-hCG水平最低,CA125水平最高(均P<0.05)。相比血清CA125、P、E2、β-hCG单项检测,4项联合检测对异位妊娠的诊断灵敏度、特异性、准确性、阳性预测值、阴性预测值更高(90.9%、98.4%、96.9%、93.8%和97.7%)(均P<0.05)。结论:早期妊娠先兆流产与异位妊娠孕妇血清β-hCG、E2、P、CA125水平差异明显,血清学联合检测对先兆流产与异位妊娠有较高的鉴别诊断价值。Objective:To investigate the value of serum HCG(human chorionic gonadotropin),E 2(estradiol 2),P(progesterone)and CA125(cancer associated glycogen 125)levels of women during the first trimester pregnancy for diagnosing ectopic pregnancy or threatened abortion.Methods:The data of 160 women during the first trimester pregnancy were analyzed retrospectively,which included 48 cases with threatened abortion(threatened group),32 cases with ectopic pregnancy(ectopic group),and 80 normal pregnant women(normal group).The serum levels of beta HCG,E 2,P and CA125 of all women were detected,and their value for diagnosing ectopic pregnancy or threatened abortion was analyzed.Results:Among the three groups,the serum P,E 2,and beta HCG levels of women in normal group were the highest and CA125 level was the lowest in ectopic group,but the serum P,E 2,and beta HCG levels of women in ectopic group were the lowest of pregnant women and CA125 level was the highest in normal group(P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combinative detection of serum levels of CA125,P,E 2and beta HCG for the diagnosing ectopic pregnancy were which were 90.9%,98.4%,96.9%,93.8%and 97.7%,respectively,which were significant higher than those of single detection of serum CA125 level,P level,E 2 level or beta HCG level(P<0.05).Conclusion:During the first trimester pregnancy,there are significant different in the levels of serum beta HCG,E 2,P and CA125 between the women with threatened abortion and the women with ectopic pregnancy.The combinative detection of serum levels of CA125,P,E 2 and beta HCG for differential diagnosing threatened abortion or ectopic pregnancy has higher value.
