丹栀逍遥散加减联合左金丸外敷涌泉穴治疗肝郁化火型失眠的临床观察

摘　　要：目的:应用丹栀逍遥散加减联合左金丸外敷涌泉治疗肝郁化火型失眠患者的临床疗效观察。方法:将68例肝郁化火型失眠患者随机分配至治疗组和对照组,每组各34例。治疗组予以口服丹栀逍遥散加减的汤药,以及每晚临睡前联合左金丸粉剂经醋调和外敷涌泉穴;对照组口服甜梦胶囊。服药时间均为上午9时前服用,晚上须于21时前服用。两组均连续服用28天。观察两组的临床疗效和症候改善情况。结果:治疗组中医证候积分临床疗效总有效率为86.67%,对照组的总有效率为67.74%(P<0.05)。治疗组的SPIEGEL量表疗效的临床总有效率为83.33%,对照组的总有效率为70.00%(P<0.05)。除此之外,治疗组在改善患者的焦虑、抑郁评分及疗效方面也有显著差异(P<0.05)。结论:丹栀逍遥散加减联合左金丸外敷涌泉穴及对照组均能改善肝郁化火型的失眠临床表现及睡眠质量,而汤药加穴位敷贴疗法更有优势,且能改善患者焦虑、抑郁的情绪。Objective:To observe the clinical effects of Danzhi Xiaoyao San(丹栀逍遥散)plus Zuojin Wan(左金丸)on insomnia of the Ganyu Huahuo type(肝郁化火型).Methods:68 patients were randomly divided into the treatment group and control group,34 cases in each.The treatment group was given Danzhi Xiaoyao San plus Zuojin Wan.The control group was given the sweet dream capsule orally.The clinical effects and symptoms were observed.Results:The clinical efficacy of TCM syndrome scores was 86.67%in the treatment group,and 67.74%in the control group.The total effective rate in SPIEGEL scale was 83.33%in the treatment group,and 70.00%in the control group.Meanwhile,it was found that there were statistically significant differences in improving HAMA and HAMD score and the total efficiency between two groups.Conclusion:Danzhi Xiaoyao San plus Zuojin Wan had an obvious effect on improving depression and anxiety.
