摘　　要：目的探讨硝酸甘油与多巴胺联合治疗慢性心衰伴发肾功能不全的临床疗效。方法选取2014年1月-2017年12月采用硝酸甘油与多巴胺持续静脉泵入治疗30例慢性心力衰竭伴肾功能不全患者为研究对象,观察患者呼吸困难、水肿、肾功能肌酐、脑利钠肽前体指标的变化。结果治疗7 d后,患者呼吸困难等症状以6 min步行试验作为评价指标、肾功能肌酐指标,脑利钠肽前体指标均较治疗前明显好转,差异有统计学意义(P<0.05)。显效21例(70%)、有效8例(26.7%)、无效1例(3.33%),总有效率为96.7%。结论硝酸甘油与多巴胺联用治疗慢性充血性心衰及伴发肾功能不全可明显改善心功能,改善及保护肾功能,安全、经济,可在临床推广使用。Objective To investigate the clinical effect of combining nitroglycerin and dopamine to treat chronic heart failure with renal insufficiency.Methods Analyzing 30 patients with chronic heart failure and renal insufficiency who are treated with nitroglycerin and dopamine from Jan.2014 to Dec.2017 will be the source of clinical data,in the way of observing patients'changes in dyspnea,edema,renal creatinine and brain natriuretic peptide precursors.Results After 7 days'treatment,using 6-minute walk test as an evaluation index to assess patients,dyspnea and other symptoms,the Cr,and the pro-BNP were significantly improved,and the differences were statistically significant(P<0.05).Excellence was achieved in 21 patients(70%),effectiveness was achieved in 8 patients(26.7%),ineffectiveness was observed in 1 patient(3.33%),and the total effective rate was 96.7%.Conclusion The combination of nitroglycerin and dopamine has significant clinical effects in treating chronic heart failure with renal insufficiency.It can obviously improve the cardiac function,as well as improving and protecting renal functions.Additionally,this treatment is safe and economical for the clinical operation.
