摘  要:目的研究去乙酰化酶SIRT1在新生儿坏死性小肠结肠炎(NEC)发病机制中的作用。方法纳入2015年3月至2019年3月于我院接受手术的28例坏死性小肠结肠炎患儿为观察组,另选取同期入院的30例嵌顿性腹股沟斜疝的新生患儿作为对照组。收集所有入选对象的肠道标本,采用蛋白免疫印迹(Western blot)法检测两组患儿肠管SIRT1蛋白的含量。同时将40只7日龄无特定病原体(SPF)级新生BALB/c小鼠随机分为造模组和对照组(每组20只),建立坏死性小肠结肠炎的小鼠动物模型,收集两组小鼠的回肠标本,采用实时荧光定量PCR法、Western blot法检测其肠道内SIRT1的mRNA及蛋白含量,并运用免疫荧光方法检测两组SIRT1的转位情况。结果观察组患儿肠管组织中SIRT1蛋白质表达水平均高于对照组,差异具有明显的统计学意义(均P<0.05)。观察组SIRT1蛋白的含量显著高于对照组(P<0.05)与对照组相比,造模组小鼠SIRT1 mRNA及蛋白表达水平均显著降低(均P<0.05),同时免疫荧光结果显示,与对照组相比,NEC小鼠肠管细胞SIRT1蛋白在细胞核表达逐渐减少,细胞质中表达明显增多。结论去乙酰化酶SIRT1可能参与NEC发病过程,临床应引起足够重视。Objective In order to study the role of deacetylase SIRT1 in the pathogenesis of neonatal necrotizing enterocolitis(NEC).Methods Twenty-eight children with necrotizing colitis who underwent surgery in our hospital from March 2015 to March 2019 were included in the observation group,and the control group of neonatal incarcerated inguinal hernia was also selected.Another group of 30 neonates with incarcerated inguinal hernia who were admitted to the hospital at the same time were selected,and the protein expression of SIRT1 in the intestinal tract of the two groups was determined by Western blotting.40 7-day-old SPF-free newborn BALB/c mice were randomly divided into model group and normal control group(20 in each group),a mouse model of necrotizing enterocolitis was established,and the ileum specimens of mice in the two groups were collected and the SIRT1 gene and protein contents were detected by real-time fluorescence quantitative PCR and the translocation of SIRT1 was detected by using immunofluorescence.Results The expression levels of SIRT1 protein in the intestinal tissue of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).Compared with the control group,the expression levels of SIRT1 mRNA and protein in the observation group were significantly decreased(P<0.05),and the results of immunofluorescence showed that compared with the control group,the expression of SIRT1 protein in the intestinal tube of NEC mice gradually decreased in the nucleus and the expression in the cytoplasm increased significantly.Conclusion The deacetylase SIRT1 is involved in the pathogenesis of NEC and should be paid enough attention in the clinical practice.
