中药治疗流行性感冒有效性和安全性的系统评价

摘　　要：目的系统评价中药对流行性感冒的治疗作用及安全性。方法计算机检索Pubmed、Embase、中国知网、万方数据库、维普数据库、中国生物医学文献数据库关于中药治疗流感的临床随机对照试验,检索时限为建库至2017年12月。文献筛选、数据提取以及偏倚风险评估由2名评价员独立完成,并对纳入的文献真实性进行确认。采用Revman5.3软件进行Meta分析。结果纳入合格研究9篇,共1603例参试者。Meta分析显示,中药临床有效率高于利巴韦林,95%CI(1.20~1.49);连花清瘟胶囊能有效缩短发热时间,作用优于奥司他韦(P=0.003);金花清感颗粒有一定退热作用(P=0.011)。麻杏石甘-银翘散能降低抗生素的使用率,作用与奥司他韦相当。结论中药能有效改善流感症状,缩短症状持续时间,对于促进病毒核酸转阴也有一定作用,安全性较高。Objective To evaluate the curative effect and safety of traditional Chinese medicine on influenza by systematic review.Methods Papers on the randomized controlled trial of influenza treatment by Chinese medicine were explored and analyzed in Pubmed,Embase,CNKI,Wanfang Data,VIP and CBM.The publication dates of the papers ranged from the origin of the databases to December 2017.Literature screening,data extraction,and risk assessment for bias were independently performed by two reviewers and the authenticity of the included literature was confirmed.Meta-analysis was done by Revman 5.3.Results A total of 1603 participants were included in 9 qualified studies.Meta analysis results showed that the clinical effective rate of TCM was higher than ribavirin(95%CI,1.20~1.49).Lianhua qingwen Capsule was better than oseltamivir in reducing fever time(P=0.003).Jinhua qinggan Granule also has a certain antipyretic effect(P=0.011).Compared with placebo,Maxingshigan-Yinqiaosan can significantly reduce the usage of antibiotics,and there was no statistical difference between Maxingshigan-Yinqiaosan and oseltamivir.Conclusion Traditional Chinese medicine can release the influenza symptoms effectively and shorten the duration time of symptoms.It can also contribute to the negative conversion of the viral nucleic acid and has comparatively high safety.
