改良Stoppa入路在髋臼及骨盆骨折手术治疗中的应用

摘　　要：目的探讨改良Stoppa入路在髋臼及骨盆骨折手术治疗的应用效果。方法39例髋臼及骨盆骨折患者,均采用改良Stoppa入路进行手术,观察手术时间、术中出血量、术后切口愈合情况和并发症发生情况,根据Matta影像学评分标准对患者髋臼及骨盆复位情况进行评价。结果所有患者均成功完成手术。手术时间(132.89±33.55)min,术中出血量(357.73±40.19)ml,术后切口均获得I期愈合。术后影像学评估显示21例骨盆骨折和16例髋臼骨折患者的复位评估为优,其余患者的复位评估为良,影像学评估优良率94.9%,HHS评分总体优良率89.7%。术后屈髋功能受限的发生率最高(12.82%),下肢深静脉血栓形成的发生率为10.25%。结论改良Stoppa入路治疗髋臼及骨盆骨折可充分暴露术野,具有良好的效果,且术后并发症少,值得推广应用。Objective To investigate the application effect of modified Stoppa approach in treatment of acetabular and pelvic fractures.Methods Thirty-nine patients with acetabular and pelvic fractures were included.They underwent surgery with the modified Stoppa approach.The operation time,intraoperative bleeding volume,postoperative wound healing and complications were observed.Acetabular and pelvic reduction was evaluated with Matta scoring criteria.Results The operations were successfully completed in all patients.The operation time was(132.89±33.55)min,operation hemorrhage was(357.73±40.19)ml,and primary healing was obtained in all incisions.Image evaluation showed that there were 16 cases of pelvic fracture and 10 cases with acetabular fracture with excellent reduction and reduction in the rest of the patients was good.The excellent and good rate was 94.8%.The incidence of limited hip function after operation was the highest(12.8%)and the lower extremity deep venous thrombosis occurred in 10.2%of patients.Conclusion The modified Stoppa approach for the treatment of acetabular and pelvic fractures can expose the operative field very well and has a good effect.It also has less postoperative complications.It is worth popularizing and applying.
