门诊专科护理小组在永久性心脏起搏器植入术后患者随访中的应用

摘　　要：目的:探讨门诊专科护理小组在永久性心脏起搏器植入术后患者随访中的应用效果。方法:将130例永久性心脏起搏器植入患者随机分为观察组67例和对照组63例,其中观察组4例失访,对照组2例失访,对照组实施常规术后随访,观察组在对照组基础上增加门诊专科护理小组随访,两组患者均于起搏器植入术后进行为期1~2年的随访。比较两组患者心功能改善情况、起搏器相关知识掌握情况、生活质量及并发症发生情况。结果:随访1年后,两组患者左心室射血分数(LVEF)均有上升,左室舒张末期内径(LVEDd)均有下降,且观察组变化幅度更加显著(P<0.05);两组相关疾病知识、康复知识及日常生活指导等评分均有上升,且观察组上升幅度更加显著(P<0.05);观察组躯体功能、心理功能、社会功能及一般认知评分较对照组明显改善(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论:门诊专科护理小组在永久性心脏起搏器植入术后随访中,能够帮助患者提高起搏器相关知识掌握水平,改善其生活质量及预后。Objective:To explore the application effect of outpatient specialized nursing group in follow-up of patients after permanent cardiac pacemaker implantation.Methods:A total of 130 patients with permanent pacemaker implantation were randomly divided into observation group with 67 cases and control group with 63 cases,including 4 cases of observation group lost to follow-up and 2 cases lost to follow-up in control group.The control group was implemented routine postoperative follow-up.The observation group added follow-up by outpatient specialized care group on the basis of control group.The patients after pacemaker implantation of two groups were followed up for 1-2 years.The improvement of cardiac function,knowledge of pacemaker,quality of life and complications were compared between the two groups.Results:After 1 year of follow-up,left ventricular ejection fraction(LVEF)increased in both groups,and LVEDd decreased in both groups,with more significant changes in the observation group(P<0.05).The scores of related disease knowledge,rehabilitation knowledge and daily life guidance increased in both groups,and the increase was more significant in the observation group(P<0.05).The scores of physical function,psychological function,social function and general cognition in the observation group were significantly improved compared to those in the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:In the follow-up after permanent cardiac pacemaker implantation,the outpatient specialized nursing team can help patients improve the level of knowledge related pacemaker,improve quality of life and prognosis.
