重建钢板内固定术可有效促进漂浮肩损伤患者的术后恢复

摘　　要：为了探讨重建钢板内固定术治疗漂浮肩损伤骨折及恢复肩关节功能恢复上的疗效,本研究收集了8例漂浮肩患者临床病例,收集的患者均进行了重建钢板内固定术治疗,患者术后3~5 d开始进行双上肢钟摆样被动功能锻炼,术后3周开始进行肩关节主动功能训练,术后9个月入院复诊进行锁骨内固定物取出术。采用Herscovici肩关节功能标准评分于术后3个月、6个月、1年时评估其治疗情况,并评估其肩关节活动度,统计治疗期间并发症发生情况。研究表明锁骨/肩胛骨骨折平均愈合时间为(8.4±1.6)周;术后3个月,患者恢复良好率为37.5%,术后6个月,患者恢复优良率为50%,术后1年为87.5%,其关节活动度依次为(117.3±3.2)°、(138.6±3.4)°、(161.5±1.2)°;仅1例患者出现三角肌肌力减弱,其余患者均未出现关节疼痛、延迟愈合、感染及内固定断裂、移位等并发症。本研究得出初步结论,重建钢板内固定术可有效改善漂浮肩损伤患者临床症状,促使其愈合及功能恢复,且未发生并发症,安全性较高。In order to explore the fracture curative effect of reconstruction plate fixation for treatment of floating shoulder injury and recovery of shoulder joint function, 8 clinical cases of floating shoulder patients were collected in this study. All the patients were treated with reconstructed steel plate internal fixation. The patients began to perform the passive function exercise of the pendulum of both upper limbs from 3 d to 5 d after surgery, and the active function training of shoulder joints was started 3 weeks after surgery. After 9 months, the patient was sent to hospital for re-examination for removal of internal fixation of clavicle. The Herscovici shoulder joint function standard score was used to evaluate the treatment at 3 months, 6 months and 1 year after surgery, as well as the shoulder joint mobility, and the complications during the treatment were counted. The average healing time of collarbone/scapula fractures was(8.4±1.6) weeks. 3 months postoperatively, patients with good recovery rate 37.5%,6 months postoperatively, patients good recovery was 50% and 87.5% after 1 year, followed by its joint mobility(117.3±3.2)°,(138.6±3.4)°,(161.5±1.2)°;only 1 patient had decreased deltoid muscle strength, while the rest did not have complications such as joint pain, delayed healing, infection, rupture of internal fixation and displacement.The preliminary conclusion of our study is that the reconstruction of plate fixation can effectively improve the clinical symptoms of patients with floating shoulder injuries, promote their healing and functional recovery, and there are no complications and the safety is relatively high.
