摘 要:目的评价外伤性晶状体脱位应用囊袋张力环手术的效果。方法回顾性分析2011年1月至2018年6月外伤性晶状体脱位或半脱位71例(71只眼)的临床资料。患者随机分为两组:对照组(34例)进行后房型悬吊人工晶状体缝合固定术,观察组(37例)进行囊袋张力环植入联合人工晶状体植入术。术后随访3~6个月。结果至随访结束,观察组的视力恢复情况优于对照组(χ2=5.456,P=0.020)。手术并发症主要包括继发性青光眼、角膜失代偿及人工晶状体偏位等,观察组发生并发症共9例,发生率为24.3%,对照组发生22例,发生率为64.7%,差异有统计学意义(χ2=9.856,P=0.002)。术后屈光度观察组为(-1.54±0.37)D,对照组为(-6.52±1.62)D,差异有统计学意义(t=7.045,P<0.001)。结论囊袋张力环用于晶状体脱位可有效提高术后视力,并发症发生率低,屈光状态较好。Objective To evaluate the efficacy of application of capsular tension ring in the treatment of traumatic luxation of lens.Methods The data of 71 eyes of 71 cases with traumatic dislocation of lens or subluxation of lens from Jan.2011 to Jun.2018 were analyzed retrospectively.All cases were divided randomly into two groups.The control group,34 cases,received intraocular lens posterior chamber suspension;the observation group,37 cases,received application of capsular tension ring and intraocular lens implantation.The followed-up time was 3-6 months.Results At the end of follow-up,the visual acuity of observation group was better than that of control group(χ2=5.456,P=0.020).Surgical complications included secondary glaucoma,corneal decompensation,uvea inflammatory response and IOL dislocations.Complications occurred in 9 cases(24.3%)in observation group,and in 22 cases(64.7%)of the control group.The difference was statistically significant between the two groups(χ2=9.856,P=0.002).After operation,the average refractive diopter of the observation group was(-1.54±0.37)D and(-6.52±1.62)D in control group.The difference was statistically significant(t=7.045,P=0.000).Conclusion Application of capsular tension ring in the treatment traumatic luxation of lens can effectively improve the visual acuity with low complication rate and better refractive diopter.
