摘　　要：目的探讨背向散射积分(IBS)对糖尿病早期视网膜病变的诊断价值。方法选取北部战区总医院自2014年6月至2015年6月收治的无糖尿病视网膜病变(NDR)的80例非胰岛素依赖性糖尿病患者(80只眼)为研究对象,根据病程分为5-NDR组(病程≤5年,n=38)与10-NDR组(病程5~10年,n=42)。选取40例同期健康者(40只眼)作为健康组。测量3组受检者视网膜中央动脉的血流动力学指标及鼻侧视网膜的IBS值,计算校正IBS值(IBS%)。结果5-NDR组与10-NDR组的阻力指数(RI)、IBS、IBS%显著高于健康组;10-NDR组的RI、IBS、IBS%显著高于5-NDR组,差异均有统计学意义(P<0.05)。5-NDR组、健康组的最大收缩期血流速度(PSV)、舒张末期血流速度(EDV)显著高于10-NDR组,差异均有统计学意义(P<0.05)。结论糖尿病患者出现早期视网膜病变时,视网膜IBS明显增高。IBS可作为早期诊断糖尿病视网膜病变的有效指标。Objective To investigate the diagnostic value of backscatter integral(IBS)in early diabetic retinopathy.Methods A retrospective study was performed on 80 cases(80 eyes)of patients with non-insulin-dependent diabetic retinopathy(NDR)who were admitted from June 2014 to June 2015.According to the course of disease,the patients were divided into 5-NDR group(course of disease≤5 years)and 10-NDR group(5 years≤course of disease≤10 years).The 40 healthy subjects(40 eyes)were selected as healthy group.Hemodynamic indexes of central retinal artery and IBS values of nasal retina in the 3 groups were measured,and the corrected IBS values(IBS%)were calculated.Results Resistance index(RI),IBS and IBS%in 5-NDR group and 10-NDR group were significantly higher than those in healthy group.RI,IBS and IBS%in the 10-NDR group were significantly higher than those in the 5-NDR group,with statistically significant differences(P<0.05).The maximum systolic blood flow velocity(PSV)and end diastolic blood flow velocity(EDV)of the 10-NDR group were significantly lower than those of the healthy group and the 5-NDR group(P<0.05).Conclusion Diabetic patients showed early retinopathy,with no positive changes in fundus,but significantly increased retinal IBS.IBS can be an effective indicator for early diagnosis of diabetic retinopathy.
