张华东教授从肝肺论治类风湿肺间质纤维化

摘　　要：类风湿关节炎(RA)是累及全身多系统的自身免疫性疾病。RA关节外受损最多的为肺部,尤以类风湿关节炎合并间质性肺炎(RA-ILD)最为常见。张华东教授为谢海洲、路志正学术经验继承人,后师从国医大师朱良春、李济仁及全国名老中医娄多峰、薛伯寿、张炳厚等,从事中医风湿病临床、科研多年,对类风湿肺间质纤维化的中医辨证治疗有独到见解。RA-ILD中医证型复杂,张华东教授认为治疗本病应从“五体痹-五脏痹”理论出发,从五脏论治类风湿肺间质纤维化,不单从肺论治。张教授临证过程中认为中医论治类风湿肺间质纤维化应该有是证用是方,不能忽略患者临床表现,对于该病表现为肝肺病变的患者,应重点从肝肺论治。Rheumatic arthritis(RA)is an autoimmune disease involving multiple systems in the whole body.Except the damage of joints in RA,lung is the very often organ involved,especially the complicated with interstitial lung disease(RA-ILD).Professor Zhang Huadong is the inheritor of the academic experiences of Xie Haizhou and Lu Zhengzhi,later is under the apprenticeship with Zhu Liangchun,Li Jren,Lou Duofeng,Xue Boshou and Zhang Binghou.He has been engaged in clinical research of rheumatic disease of TCM for years.He has the unique understanding of rheumatoid interstitial fibrosis(RIF)in TCM differentiation.The syndromes of RA-ILD are complicated in TCM.Professor Zhang Huadong believes that this disease should be on the base of the theory of“five-body bi syndrome and five-zang bi syndrome”.RIF is treated in terms of the theory of five zang organs rather than based on lung theory in TCM.In clinical treatment,RIF should be treated in accordance with the differentiated syndromes and the corresponding formulas are selected.The clinical manifestation should not be neglected.For the patients with the manifestation of liver-lung disorders,thus,the treatment should focus on the the liver-lung theory.
