苓桂术甘汤联合前列地尔治疗肝硬化腹水疗效及对患者生活质量的影响

摘　　要：目的探讨苓桂术甘汤联合前列地尔治疗肝硬化腹水的疗效及对患者生活质量的影响。方法选取肝硬化腹水患者150例,根据随机数字表法将患者分为对照组、观察1组和观察2组,每组均50例。对照组给予常规治疗,观察1组在对照组的基础上联合前列地尔进行治疗,观察2组在对照组的基础上采用苓桂术甘汤联合前列地尔进行治疗,3组均治疗4周。比较3组患者的临床疗效、中医症状积分、肝功能指标、SF-36评分以及不良反应。结果观察2组患者的临床总有效率明显高于观察1组和对照组(P均<0.05)。治疗后,观察2组的各项中医症状积分均明显低于观察1组和对照组(P均<0.05),各项肝功能指标均明显低于观察1组和对照组(P均<0.05),生理职能、健康状况、躯体疼痛、情感职能评分均明显高于观察1组和对照组(P均<0.05),3组患者均未出现明显不良反应。结论苓桂术甘汤联合前列地尔治疗肝硬化腹水具有较好的临床疗效,可有效改善患者的临床症状、肝功能和生活质量,且安全。Objective It is to investigate the efficacy of Lingguishugan decoction combined with alprostadil in the treatment of ascites due to cirrhosis and its effect on patients’quality of life.Methods A total of 150 patients with cirrhosis and ascites were divided into control group,observation group I and observation group II according to the random number table method,50 cases in each group.The control group received routine treatment,observation group I was treated with alprostadil on the basis of the control group,and observation II were treated with Lingguizhugan decoction combined with alprostadil on the basis of the control group.All the groups were treated for 4 weeks.The clinical efficacy,TCM symptom scores,liver function indicators,SF-36 scores,and adverse reactions were compared between the three groups.Results The total clinical effective rate of the observation group II was higher than that of the observation group I and the control group(P<0.05).After treatment,the TCM symptom scores of the observation group II were lower than those of the observation group I and the control group(P<0.05).After treatment,the liver function indexes of the observation group II were lower than those of the observation group I and the control group(P<0.05).After treatment,the physiological functions,health status,physical pain,and emotional function scores of the observation group II were higher than those of observation group I and control group(P<0.05),no significant adverse reactions occurred in the three groups.Conclusion Lingguishugan decoction combined with alprostadil has a good clinical effect in the treatment of ascites due to cirrhosis,which can effectively improve the clinical symptoms,liver function and quality of life of patients without obvious adverse reactions.
