平悸饮合参麦注射液治疗急性冠脉综合征伴顽固性室性心律失常疗效及对相关炎性因子水平的影响

摘　　要：目的探讨平悸饮合参麦注射液治疗急性冠脉综合征(ACS)伴顽固性室性心律失常疗效及对相关炎性因子指标水平的影响。方法将94例ACS伴顽固性室性心律失常患者随机分为2组,每组47例。对照组给予西医常规治疗,观察组在此基础上加用平悸饮合参麦注射液治疗,疗程均为14 d。观察2组治疗后室性早搏(PVC)发作次数、短阵性室性心动过速(PST)发作次数、QRS波时限、QT间期、Tp-e间期、矫正Tp-e间期/QT间期、LOWN分级及血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、IL-8、CD40配体水平变化情况,统计2组临床疗效和不良反应发生情况。结果2组治疗后PVC发作次数、PST发作次数均显著减少(P均<0.05),QRS波时限、QT间期、Tp-e间期、矫正Tp-e间期/QT间期均显著缩短(P均<0.05),血清CRP、TNF-α、IL-6、IL-8、CD40配体水平均显著降低(P均<0.05),LOWN分级均显著改善(P均<0.05),且观察组改善情况均显著优于对照组(P均<0.05)。观察组治疗总有效率显著高于对照组(P<0.05),2组不良反应发生率比较差异无统计学意义(P>0.05)。结论平悸饮合参麦注射液治疗ACS伴顽固性室性心律失常可安全有效降低心律失常发作频率,减轻病情,可能与明显下调相关炎性因子水平有关。Objective It is to investigate the effect of Pingjiyin decoction combined with Shenmai injection on acute coronary syndrome(ACS)with refractory ventricular arrhythmia and its influence on the level of related inflammatory factors.Methods 94 patients with ACS with refractory ventricular arrhythmia were randomly divided into 2 groups,47 cases in each group.The control group was given conventional treatment with western medicine.The observation group was treated with Pingjiyin decoction and Shenmai injection on the basis of the control group.The course of treatment was 14 days.The changes of attack times of premature ventricular contraction(PVC)and paroxysmal ventricular tachycardia(PST),QRS wave duration,QT interval,Tp-e interval,corrected Tp-e interval/QT interphase,LOWN classification and the levels of serum C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),IL-8,CD40 ligand after treatment were observed,the clinical efficacy and adverse reactions occurred were assessed in the two groups.Results After treatment,the attack times of PVC and PST in the two groups were significantly reduced(P<0.05),the QRS wave duration,QT interval,Tp-e interval,and corrected Tp-e interval/QT interval were significantly shortened(P<0.05),levels of serum CRP,TNF-α,IL-6,IL-8,CD40 ligand were significantly decreased(P<0.05),LOWN classification was significantly improved(P<0.05),and the improvements of these indexes in the observation group were better than those in the control group(P<0.05).The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Pingjiyin decoction combined with Shenmai injection can safely and effectively reduce the frequency of arrhythmia and reduce the severity of ACS with refractory ventricular arrhythmia,which may be related to the significant down-regulation of inflammatory factors.
