摘　　要：目的:探讨三维对比增强磁共振血管造影(3D-CE-MRA)诊断下肢动脉硬化闭塞症(ASO)的价值。方法:回顾性选取130例下肢ASO患者,均行3D-CE-MRA和数字减影血管造影(DSA)检查。以DSA为金标准,分析3D-CE-MRA示盆部、大腿和小腿部位的血管狭窄分级情况,并观察原始图像质量。结果:130例患者中共选取动脉段780段,应用Kappa一致性检验对3D-CE-MRA与DSA判断下肢动脉狭窄程度比较,二者一致性极佳(Kappa=0.935,P<0.05);以DSA为金标准,3D-CE-MRA判断下肢动脉狭窄程度符合率为94.87%,其差异无统计学意义(x^2=0.788,P>0.05)。780段下肢动脉图像中优质图像690段,良好图像90段,优良率为100.00%,3D-CE-MRA与DSA图像质量优良率比较无统计学差异;其中下肢动脉病变中,以股动脉病变占37.05%,其次为腘动脉和胫前动脉分别占23.84%和20.89%。结论:3D-CE-MRA诊断下肢ASO有较好的应用价值,值得临床使用。Objective:To explore the value of three-dimensional contrast-enhanced magnetic resonance angiography(3D-CE-MRA)in the diagnosis of arteriosclerosis obliterans(ASO)of lower extremities.Methods:130 patients with ASO of lower extremity who admitted to Nangang Branch Hospital of the Heilongjiang Provincial Academy of Traditional Chinese Medicine were retrospective selected in this research,and all of them underwent the examinations of 3D-CE-MRA and digital subtraction angiography(DSA).DSA was used as the gold standard,and the classifications of vascular stenosis in the pelvis,thigh and calf as 3D-CE-MRA were analyzed,and the original image qualities of them was further observed.Results:A total of 780 arterial segment were selected from 130 patients.The results of Kappa consistency,which was used to compare 3D-CE-MRA and DSA in diagnosing the degree of lower extremity arterial stenosis,indicated that the consistency between two methods were well(Kappa=0.935,P<0.05).With DSA as the gold standard,the coincidence rate of 3D-CE-MRA for the degree of lower extremity arterial stenosis was 94.87%,and the difference of coincidence rate between DSA and 3D-CE-MRA was no significant(x^2=0.788,P>0.05).There were 690 segments high-quality images in the images of 780-segment lower extremity arteries,and 90 segments were good images,and excellent and good rate of images was 100.00%.There was no statistically significant difference between 3D-CE-MRA and DSA for excellent and good rate of image quality.In the lesions of lower extremity artery,the femoral artery lesion accounted for 37.05%,and popliteal artery accounted for 23.84%,and anterior tibial artery accounted for 20.89%,respectively.Conclusion:3D-CE-MRA examination has a good application value in diagnosing ASO of lower extremities and it is worthy in clinical application.
