人工鼻在颅脑损伤机械通气患者中的应用效果观察

摘　　要：目的探讨人工鼻在颅脑损伤机械通气患者气道管理中的应用效果。方法将120例经人工气道机械通气的颅脑损伤患者随机分为对照组和研究组,对照组60例利用呼吸机本身配置的加温湿化装置进行湿化;研究组60例,在人工气道管口连接一次性复合人工鼻进行气道湿化过滤,对2组在机械通气过程中痰液黏稠度、气道阻力、痰培养阳性率、护理工作量等方面进行比较。结果研究组痰液湿化效果优于对照组,痰培养阳性率及日均护理项目少于对照组,气道阻力增加病例数多于对照组。结论颅脑损伤机械通气患者应用人工鼻后,不仅能够有效保证呼吸道温度、湿度,而且可截留人工气道及呼吸机管路中的细菌,降低VAP发生率;不仅可有效提高颅脑损伤机械通气患者的护理质量,同时还可增加工作效率,提高救治率,缩短患者的住院时间,降低住院费用。Objective To investigate the artificial nose in airway management of trantic brain injury patients with mechanical ventilation application effect.Methods 120 patients with artificial airway patients receiving mechanical ventilation were randomly divided into control group and study group,60 cases in the control group using the respirator which configuration warm-up humidification device for humidification;the study group 60 cases,in artificial airway nozzle connected with disposable composite artificial nasal airway humidification of patients in two groups of filter,mechanical during ventilation,airway resistance,sputum viscosity of sputum culture positive rate,nursing burden and other aspects of comparison.Results The study group consisted of sputum humidification effect is better than that of control group.Sputum culture positive rate and daily care project are less than that in the control group.The number of airway resistancing increased cases is more than the control group.Conclusion The patients with mechanical ventilation after application of artificial nose,not only can ensure the respiratory temperature and humidity,retailing the artificial airway and ventilator pipe in bacteria,reducing the incidence rate of VAP,It not only effectively improves the nursing quality of patients with craniocerebral injury with mechanical ventilation,but also increases the working efficiency,improves the treatment rate,shorens the hospitalization time of patients,and reduces the hospitalization cost.
