摘 要:目的研究硫酸镁对重症手足口病患儿血清神经元特异性烯醇化酶(NSE)的影响。方法选取2016-03—12郑州儿童医院重症监护室收治的重症手足口病伴血清NSE异常且处于急性期的患儿100例为研究对象,采用随机数字表法分为对照组和治疗组(A、B、C组),每组25例,均常规给予抗病毒、抗感染、对症等常规治疗,对照组给予神经节苷脂2 mL/d,A组给予小剂量硫酸镁[40 mg/(kg·d)],B组给予大剂量硫酸镁[80 mg/(kg·d)],C组给予神经节苷脂(2 mL/d)联合小剂量硫酸镁[40 mg/(kg·d)],7 d为1个疗程。对比4组治疗7 d后临床疗效,以及治疗前、治疗3 d、治疗7 d血清NSE的变化。结果治疗7 d后,4组临床疗效比较差异无统计学意义(P>0.05)。治疗前,4组血清NSE水平比较差异无统计学意义(P>0.05)。治疗3 d、7 d,A、B、C组血清NSE较对照组降低,差异有统计学意义(P<0.05);B、C组血清NSE降低水平较A组差异有统计学意义(P<0.05)。4组治疗7 d血清NSE下降水平比较,差异有统计学意义(P<0.05)。结论硫酸镁可有效降低重症手足口病患儿血清NSE,有较好的脑保护作用,对病情的转归有积极作用。Objective To study the effect of magnesium sulfate on serum NSE in children with severe HFMD.Methods In this study,100 cases of severe HFMD with abnormal serum NSE and in the acute phase were selected as the study subjects in the intensive care unit of Zhengzhou Children's Hospital from March 2016 to December 2016,and were divided into the control group and the treatment group(group A,B and C),with 25 cases in each group.Conventional treatment of antiviral,anti-infection,and symptomatic treatment was given,and the control group was given 2 mL/d of ganglioside.A group was given a small dose of magnesium sulfate(40 mg/(kg·d));Large doses of magnesium sulfate(80 mg/(kg·d))were given in group B;In group C,ganglioside lipids(2 mL/d)were combined with small doses of magnesium sulfate(40 mg/(kg·d)).7d is one course of treatment.The clinical efficacy and serum NSE changes were compared in 4 groups.Results After treatment of 7d,the clinical efficacy of the four groups was not statistically significant(P>0.05).Before treatment,the serum NSE level of the four groups was not statistically significant(P>0.05).In the treatment of 3d and 7d,the serum NSE of group A,B and C was lower than the control group,and the difference was statistically significant(P<0.05).The levels of serum NSE in group B and C were lower than that in group A,and the difference was statistically significant(P<0.05).There was a statistically significant difference between the four groups of children treated with 7d compared with the pre-treatment serum NSE level(P<0.05).Conclusion Magnesium sulfate can effectively reduce serum NSE in children with severe HFMD,and it has a good brain protection effect,and it has a positive effect on the prognosis of the disease.
