基层医院感染病原菌分布及耐药性分析

摘　　要：目的:探讨基层医院感染病原菌的临床分布及耐药情况。方法:采用纸片扩散法进行药物敏感性试验,对病原菌的耐药性进行统计分析。结果:2017年3月-2018年9月共分离出非重复菌株171株,其中革兰阳性菌54株(31.6%),革兰阴性菌117株(68.4%)。菌株分布前三位的分别是大肠埃希菌,铜绿假单胞菌和鲍曼不动杆菌。MRSA的检出率为59%。大肠埃细菌、克雷伯菌属(肺炎克雷伯菌和产酸克雷伯菌)和奇异变形杆菌产ESBLs菌株检出率分别为44.4%、36.4%和50.0%。肠杆菌科细菌中CRE的检出率18.0%。铜绿假单胞菌对碳青霉烯类较敏感,而鲍曼不动杆菌对碳青霉烯类耐药严重。结论:基层医院感染病原菌耐药情况严重,应加强对细菌耐药性的监测及防控措施,协助基层医疗机构提高诊疗水平,促进抗菌药物的合理使用。Objective:To investigate the clinical distribution and drug resistance of pathogenic bacteria in grassroot hospital infection.Methods:the drug sensitivity test was carried out by disk diffusion method,and the drug resistance of pathogens was analyzed statistically.Results:From March 2017 to September 2018,171 non-repetitive strains were seperated,of which 54(31.6%)were Gram-positive and 117(68.4%)Gramnegative.The first three strains were Escherichia coli,Pseudomonas aeruginosa and Acinetobacter baumannii.The detection rate of MRSA was 59%.the detection rate of Escherichia coli,Klebsiella pneumoniae(Klebsiella pneumoniae and Klebsiella acidogenes)and Proteus mirabilis producing ESBLs strai were 44.4%,36.4%and 50.0%,respectively.The detection rate of CRE in Enterobacteriaceae was 18.0%.Pseudomonas aeruginosa was sensitive to carbapenems while Acinetobacter baumannii was highly resistant to carbapenems.Conclusion:The drug resistance of pathogenic bacteria in primary hospital infection is serious,so it is necessary to strengthen the monitoring and control measures of bacterial drug resistance,to help the primary medical institutions to improve the level of diagnosis and treatment,and to promote the rational use of antibacterial drugs.
