摘　　要：目的探讨会阴黏膜小角度指引性切开在第二产程中的应用效果。方法选择2016年3月至2018年3月阴道分娩产妇62例作为研究对象,随机分为对照组(n=31)和观察组(n=31)。对照组给予传统会阴切开术分娩,观察组采用会阴黏膜小角度指引性切口分娩。比较两组分娩后出血、损伤、感染、疼痛、切口愈合不良发生率,记录两组切口出血量、缝合时间及直观模拟量表(VAS)评分、住院时间、新生儿出生体质量,采用性功能评价量表(BISF-W)对两组分娩前、后性生活质量进行评估。结果观察组分娩后出血、损伤、感染、疼痛、切口愈合不良发生率,均低于对照组(P<0.05);观察组切口出血量、缝合时间及VAS评分、住院时间,均低于对照组(P<0.05);观察组分娩后性欲、性唤起、性高潮、性心理及异常症状得分均高于对照组(P<0.05)。结论将会阴黏膜小角度指引性切开用于阴道分娩产妇第二产程中能减轻分娩损伤,提高切口愈合率,促进产妇分娩后恢复,能提高夫妻生活质量,值得推广应用。Objective To explore the application effect of perineal mucosal small angle guided incision in the second stage of labor.Methods A total of 62 cases of vaginal delivery women from March 2016 to March 2018 were selected as the subjects and randomly divided into control group(n=31)and observation group(n=31).The control group was given traditional perineal incision delivery,while the observation group was given small angle perineal mucosa guided incision delivery.The incidence of bleeding,injury,infection,pain and wound healing after delivery were compared between the two groups,the incision bleeding volume,suture time,visual analogue scale(VAS)score,hospitalization time and neonatal birth weight were recorded,and sexual function assessment scale(BISF-W)was used to assess the quality of life in two groups before and after delivery.Results The incidence of postpartum hemorrhage,injury,infection,pain and poor wound healing in the observation group were lower than those in the control group(P<0.05).The incision bleeding volume,suture time,VAS score and hospitalization time in the observation group were lower than those in the control group(P<0.05).The scores of sexual desire,arousal,orgasm,sexual psychology and abnormal symptoms in the observation group were higher than those in the control group(P<0.05).Conclusion The application of small-angle guided vaginal mucosa incision in the second stage of vaginal delivery can alleviate the birth injury,improve the incision healing rate,promote the recovery of puerpera after delivery,and improve the quality of life of husband and wife,which is worthy of popularization and application.
