摘　　要：目的研究阴式子宫切口瘢痕妊娠病灶清除术治疗剖宫产切口瘢痕妊娠疗效。方法选择本院2014年3月至2018年6月接诊的剖宫产切口瘢痕妊娠患者42例,随机分为对照组和实验组,各21例。对照组采取药物保守治疗,实验组采取阴式子宫切口瘢痕妊娠病灶清除术治疗。比较两组治疗效果,包括妊娠组织物清除、出血量、住院时间、并发症。结果实验组治愈率为95.2%,明显高于对照组(P<0.05);实验组平均出血量(114.4±23.7)ml,并发症发生率4.8%,住院时间(5.4±1.5)d,各项指标均明显低于对照组(P<0.05)。结论对剖宫产切口瘢痕妊娠采取阴式子宫切口瘢痕妊娠病灶清除术,能完全清除病灶,且创伤小利于恢复,不仅住院时间短、还可明显降低术后并发症,值得临床应用。Objective To study the effect of vaginal uterine incision scar pregnancy focus clearance in the treatment of cesarean scar pregnancy.Methods 42 cases of cesarean scar pregnancy from March 2014 to June 2018 were randomly divided into control group and experimental group,with 21 cases in each group.The control group was treated with drug conservative treatment,while the experimental group was treated with vaginal uterine incision scar pregnant lesion clearance.The therapeutic effects of the two groups were compared,including tissue removal,bleeding volume,hospital stay and complications.Results The cure rate of the experimental group was 95.2%,which was significantly higher than that of the control group(P<0.05);the average amount of bleeding in the experimental group was(114.4±23.7)ml,the incidence of complications was 4.8%,and the length of hospitalization was(5.4±1.5)days.The indexes of the experimental group were significantly lower than those of the control group(P<0.05).Conclusion Vaginal uterine incision scar pregnancies for cesarean scar pregnancies can completely remove the lesions,and the trauma is small and conducive to recovery.It can not only shorten the hospital stay,but also significantly reduce the postoperative complications,which is worthy of clinical application.
