鼻内镜上颌窦自然口扩大联合下鼻道开窗治疗霉菌性上颌窦炎的临床观察

摘　　要：目的探讨鼻内镜上颌窦自然口扩大联合下鼻道开窗治疗霉菌性上颌窦炎临床效果。方法选取2016年1月至2018年1月我院收治的96例霉菌性上颌窦炎患者,均为单侧,根据不同治疗方法分为两组,对照组(n=48)接受鼻内镜上颌窦自然口扩大治疗,观察组(n=48)再对照组基础上接受下鼻道开窗治疗,对比两组患者临床疗效和术后恢复时间以及残留霉菌率。结果两组患者临床疗效对比95.83%vs 81.25%差异明显,具有统计学意义(P<0.05)。两组患者术后恢复时间对比差异明显,具有统计学意义(P<0.05)。两组患者残留霉菌率对比4.17%vs 18.75%差异明显,具有统计学意义(P<0.05)。结论鼻内镜上颌窦自然口扩大联合下鼻道开窗治疗霉菌性上颌窦炎治疗效果更好,可缩短患者恢复时间,并降低复发率,值得临床推广。Objective To investigate the clinical effect of endoscopic maxillary sinus ostium enlargement combined with fenestration of inferior nasal meatus in the treatment of mycotic maxillary sinusitis.Methods 96 patients with mycotic maxillary sinusitis admitted to our hospital from January 2016 to January 2018 were selected and divided into two groups according to different treatment methods.The control group(n=48)received nasal endoscopic enlargement of the natural mouth of the maxillary sinus.The observation group(n=48)received fenestration of the lower nasal meatus on the basis of the control group.The clinical efficacy,recovery time and residual of the two groups were compared.Mold rate.Results The clinical efficacy of the two groups was significantly different(95.83%vs 81.25%,P<0.05).There was significant difference in recovery time between the two groups(P<0.05).The residual fungi rate of the two groups was 4.17%vs18.75%.The difference was significant(P<0.05).Conclusion Endoscopic maxillary sinus ostium enlargement combined with lower nasal meatus fenestration is more effective in the treatment of mycotic maxillary sinusitis.It can shorten the recovery time of patients and reduce the recurrence rate,which is worthy of clinical promotion.
